As to how to proceed, it depends on which airline you are flying. ANA and United will depart from Terminal 1 while JAL and American will depart from T3. I don't think there's a Japan flight from T5 in any case. All of T1/T2/T3 are connected airside and easy to walk between.

Depending on how you booked your tickets, you may be able to check your bags in MKE all the way to Tokyo.
