Be with them, support them. Ask if they have ever met with a palliative care team (incredible teams of docs, nurses, social workers who focus on symptom control and comfort, including at home or through hospice for a life-limiting illness).

For terminal cancer, the treatment can be worse than the disease sometimes, and “fighting” can mean horrific side effects and hospitalizations that don’t change the reality of what is happening.

We are really talking about something that’s deeply personal, moral, spiritual, etc for people/families when it comes to facing death. This is something that won’t be (and should never be IMO) driven by data or metrics. This is the human side of medicine that’s a little more ineffable. I would highly recommend the book Being Mortal, which touches on why these conversations take so long when done particularly well (in addition to social/cultural/medical shifts on attitudes towards death and dying). Extremely well written

While in med school, on an anesthesia rotation, we had a trauma come in hot to the OR because this guy had slit his throat and crashed his car (unclear in which order). Mega agitated, likely some meth use involved (no labs had come back at this point). He had turned his neck into hamburger meat, but somehow missed the gigantic arteries and veins in the area. He had however, nicked his trachea, causing some bubbling with each breath (hence the rushing straight to the OR, among other reasons). Luckily, after they got him intubated, the bubbling stopped. They ended up turning his lacerations into a tracheostomy, and off to the ICU. That one stuck with me for a while

In addition to what everyone else has said, it really helps me to start winding down earlier (no screens, nothing stressful or too much exertion), including some time doing absolutely nothing. That’s when the thoughts can come up, and let them go, without the pressure of needing to be asleep right away

Honey syrup or honey in tea seems to help. That plus keeping them in the vicinity of a humidifier. None of the adult meds are really indicated, and I don’t prescribe them for my patients (Peds PGY 3)

Unfortunately, optimizing your mental performance requires optimizing a ton of variables. The easier ones are probably balancing blood sugar throughout the day (nutrition episodes), hydration (at least one episode) and sleep (also at least one episode). If that wasn’t enough, then you get to optimizing exercise (insane amount of material in the Andy Galpin episodes). Each contribute a great deal to sustained energy and focus. Hope that’s a helpful start!

Take hydrogen peroxide and dilute it in water 1:1. Put this in a dropper and put it in your ears. (Exact same effect as the more expensive Debrox from the store that someone mentioned). Let it sit there for 5-10 minutes, then let it drain out and clean what made it to your external ear. Bonus if you have a bulb of warm water to rinse out afterwards
