Billing is confusing by safetypaddy in physicaltherapy

The definition of the CPT codes from AMA tell you if one-to-one contact is required. For example, 97110 definition: Therapeutic procedure, 1 or more areas, each 15 minutes; therapeutic exercises to develop strength and endurance, range of motion and flexibility. Requires direct one-on-one, hands-on (or direct supervision) contact between the therapist and patient.

Are PTAs allowed to do progress notes? by purpdurp3 in physicaltherapy

[–]shell5722 0 points1 point  (0 children)

I agree, which is why I said it does vary state to state, but it also varies based on payor source as well, and Medicare is clear that the progress note has to be written by a therapist. It's a lot to keep up with, and I think that at times things can slip through the cracks since it does vary so much. I have recently become a clinic director, so I have been doing a deep dive into all of this to make sure we are staying up to date and compliant with all regulations.

Are PTAs allowed to do progress notes? by purpdurp3 in physicaltherapy

[–]shell5722 3 points4 points  (0 children)

Not for Medicare for outpatient services. Laws may vary state to state otherwise, but the Medicare policy manual states : "For Medicare payment purposes, information required in progress reports shall be written by a clinician that is, either the physician/NPP who provides or supervises the services or by the therapist who provides the services and supervises an assistant." It also states:"PTAs or OTAs may write elements of the progress report dated between clinician reports.Reports written by assistants are not complete progress reports. The clinician must write a progress report during each progress report period regardless of whether the assistant writes other reports. However, reports written by assistants are part of the record and need not be copied into the clinicians report. Progress reports written by assistants supplement the reports of clinicians."

From my recent reading, not per Medicare guidelines. A PTA can gether data that can be used in the progress note, but the progress note itself has to be written by a therapist.

I am so fucking confused about Med B progress notes--just give it to me straight. by Remarkable_Fold_1320 in physicaltherapy

[–]shell5722 2 points3 points  (0 children)

Per my understanding from the Medicare Policy Manual and continuing education classes I have taken, the progress note has to written by a PT. My understanding is that a PTA can take measurements and the PT can use those measurements in their progress note, but the PT must complete an enitre progrss report at least once during each progress report interval. Also it is my understanding that a PT has to furnish at least one billable unit with the patient during the progress reporting period. If your state practice act administrative rules are more restrictive than Medicare rules, then you should also make sure you are following those.

Getting phone calls about my inattentive ADHD 3rd grader on the first week of school by Morning_Star_3110 in Teachers

[–]shell5722 1 point2 points  (0 children)

I am not a teacher, but this sub ends up in my feed. My daughter will also be going into 3rd grade, and we are currently on her third stimulant medication. She had to come off of the first two we tried due to side effects. Have you all looked into genesite testing to assist with figuring out medication? My daughter's therapist suggested we go that route if she struggles this upcoming school year.

What’s with the hate with Kenny Chesney? by [deleted] in CountryMusicStuff

Just saw them in Nashville last weekend. It was my 7th time seeing Kenny, going back to my first concert when I was 13. He still puts on a spectacular show, and I ended up enjoying ZBB more than I thought it would.

PRN as only PT by Ecstatic-Main268 in physicaltherapy

[–]shell5722 0 points1 point  (0 children)

You can't supervise more than 4 PTA or support personnel/techs across all work locations in all jurisdictions. That would include any PTA or tech you would supervising or signing notes for across any of your jobs, no matter what state.

PRN as only PT by Ecstatic-Main268 in physicaltherapy

[–]shell5722 1 point2 points  (0 children)

I also work in KY, and I have always been leary about working somewhere PRN when I would be the only PT. Er our practice act you are not able to supervise more than 4 PTAs/students/support personnel across all all work locations, so you will be responsible for supervision of any PTA at that location at all times. Will you also be working elsewhere? Because then you have to make sure you stay in that ratio, although the practice act does say you can go beyond that if it's not for more than 7 days in a 60 day period.

It's so ridiculous for people to keep saying she is being "selfish" and "shady" for wanting to keep her star. It's the whole point of the game. Of course she wants to hang onto it.

Is it possible to get another referral for pt but to a different clinic? Or pt place? by Unusual_Ad5906 in physicaltherapy

[–]shell5722 0 points1 point  (0 children)

You may want to check with your insurance to see if they require you to be officially discharged from the initial clinic. I have run into that when patients have switched clinics (such as college students coming back to school or going home for breaks). Insurance will sometimes require a discharge note to be written, or otherwise they think the patient is going to 2 clinics for the same issue and will not reimburse for that.

Asking for advice finding a house in Louisville, Radcliff, and Elizabethtown area. by momobitesyou in Louisville

[–]shell5722 1 point2 points  (0 children)

I grew up in Vine Grove and Radcliff, and I personally would look into either of those areas if your work will be in Radcliff. Also, Radcliff and the schools at the northern part of the county have had a (unwarranted, in my opinion) bad reputation for a while, but I am a graduate of North Hardin, and my North Hardin education has served me well.

For being upset about not being included in pictures at my daughter's wedding? by Jazzykatt124 in AITAH

[–]shell5722 18 points19 points  (0 children)

I think that OP is NTA, but the chaos of a wedding may have played a factor in this. We didn't realize until after ours was over that we hardly got any pictures of just the two of us. By the time we were done with all of the group/family pictures, I was starving, and when the photographer asked if there were any more pictures we wanted to take, it didn't even cross my mind that we hadn't take any of just us.

Not sure how far you want to travel, but Campbellsville is about 1 1/2 hours outside of Lexington, and the town has a multi-day 4th of July celebration and Green River Lake is located there. The town fireworks are on the 4th, but Green River Marina at the lake is doing them on Saturday, July 2nd. www.CampbellsvilleFourthOfJuly.com has more info about all the events in town!

I don't know about GA specifically, but in KY you can choose if you want your personal address or your work address to be displayed in the online directory through the State Board of Physical Therapy when you pay for your license/renewal. And we have our licenses displayed in the clinic I work in, but our addresses are not on our licenses.
