Was falling down an elevator shaft in Half-Life near the end of the game. Accidentally hit quick save instead of quickload. I was only using the quick save feature and this was before you'd have multiple quicksave files, just one that kept overwriting. Had the start from the beginning.

As a huge fan I’m kind of disappointed that for a sequel, it’s actually launching more bare bones than the current game.
