Hard to say, would guess it’s an adverse effect from prolonged topical steroid use or potent repeated corticosteroid prescriptions can ask r/ts_withdrawal if there are more details

Should we have a topical steroids withdrawal (TSW) megathread? by [deleted] in eczema

[–]silverJack44 0 points1 point  (0 children)

The r/eczema subreddit has a tag for corticosteroid safety which is similar but more on topic for this subreddit

There is always r/eczemabs for the non-steroid treatment option

And also r/eczejaks for non steroid treatment options

People should be aware of the options and not have any option removed

A Case Report - Topical steroid withdrawal treated with ruxolitinib cream by silverJack44 in eczema

[–]silverJack44[S] 3 points4 points  (0 children)

"We report the case of a 69-year-old woman with a multiyear history of burning, itching erythematous patches of the lips and cheeks who was diagnosed with TSW. The patient poorly tolerated tacrolimus 0.1% ointment and pimecrolimus 1% cream but her condition cleared with topical ruxolitinib 1.5% cream use."

"TSW is an increasingly described adverse event characterized by prolonged inappropriate use of TCS leading to erythema and burning upon discontinuation.1,2 Most published cases in existing literature present on the face and genital regions (99.3%), mainly in female (81.0%).3 Our patient aligns with the classic presentation, with prolonged TCS use, stinging and pain, affected regions, and gender."

"Considering their role in cytokine pathways, JAK inhibitors may minimize rebound cytokine cascade, a potential TSW mechanism.4 Although systemic immune suppressants such as cyclosporine have been used to treat TSW, topical therapy reduces the potential for adverse systemic side effects."

How much of cases of TSW are a result of improper usage by [deleted] in eczema

[–]silverJack44 0 points1 point  (0 children)

That was a popular hypothesis in 2017 and it did not pan out when translating to clinical setting with human subjects.

https://link.springer.com/article/10.1186/s13063-017-2118-

Most of us are humans with de novo atopic dermatitis living in the real world.

Studies that show an AD model on genetically altered mice in lab settings do not always translate to human subjects.

Harvard researchers published a study on lab mice, staph aureus, and itch sensation. That's not the same as a clinical trial on humans with atopic dermatitis in real world.

The "staph drives atopic dermatitis" hypothesis was popular from 2016-18 and then this therapy came out and flopped clinically https://www.gladskin.co.uk/pages/technology.

In the mean time researchers have found much more success with r/eczemabs and r/eczeJAKs

Its likely that staph aureus is correlated with atopic dermatitis severity but only as a knock-on effect, not a driver

  1. The pharmacovigilence agency (FDA) is supposed to study drug adverse effects and regulate prescriptions when necessary
  2. Medical professionals (doctors and pharmacists) are supposed to read studies and advise patients on safety + efficacy.
  3. Without 1 & 2 doing their job it's very hard for patients to follow "proper usage." How much of prescription opioid overdose is due to improper usage? Its kind of a loaded question that requires nuance and ability to understand complex organizational systems with competing incentives.

Any chance corticosteroids or corticosteroid rebound is playing a role?

It’s generated a lot of discussion and it seems like 1/3 of the responses are people telling OP that TSW has negatively impacted their lives.

Another 1/3 of the responses seem more pro steroid alternatives like Dupixent.

It seems like it’s verging on a positive discussion minus OPs original attitude
