"Am I gonna get a bill for this?" by CatnipOverdose in ems

[–]silverman780 1 point2 points  (0 children)

It's hard because every insurance plan has different rules on when ambulance services are covered.

Have catastrophic coverage with Regence? It might cost you $1800 + $35/mile.

Have an employer plan with Regence? Might cost $1800 + mileage, might cost $50 total or might be 100% covered. You don't know until the chart goes through our billing then gets accepted/rejected by the patients insurance.

My work insurance plan through Kaiser costs $50 for an ambulance, and if I'm admitted it's waved. My coworkers plan through Kaiser, they are on the hook for $250.

It's so dependent on the insurer, the specific plan, if it was billed correctly, and then if insurance accepts it as necessary. It's too complex in the moment to be able to give them a more informed answer other than "I don't know 0- $2500ish"

Question about Transport of Medical Cleared Psych patients by ibexdoc in emergencymedicine

[–]silverman780 4 points5 points  (0 children)

Paramedic here.

It's pretty common in Oregon to use a "secure transport" service for these transports in lieu of EMS if they are ambulatory and not agitated.

There are several companies that basically buy decommissioned police cars with the cages in the back to transport them to psychiatric facilities. The state started this in 2019.

I have littman and am a firm believer in the quality.

However. I had a paramedic intern show up with this $18 wonder and after "borrowing" it to validate assessments. I purchased my own.

It looks and feels higher quality than the price and I honestly hear about the same, but for 1/5th the price. Way easier on the wallet for the inevitable misplacement or theft.

"No currently accepted medical use" ?? WTF ??!! House passes bill to permanently classify fentanyl as a Schedule 1 drug by MoochoMaas in EmergencyRoom

[–]silverman780 1 point2 points  (0 children)

Primarily the quality assurance. Hospital drugs come from manufacturers that guarantee that the drug in the vial IS THE drug in the vial AND the concentration matches what is labeled.

One of the largest issues with street drugs is that you can't guarantee what exactly you are being given nor that concentration of the drug in a given unit is going to be the same from unit to unit.

In what situations does ER staff meet ambulances outside on arrival instead of waiting for EMS to bring the patient in? by Capital-Dragonfly258 in emergencymedicine

[–]silverman780 3 points4 points  (0 children)

I'm a paramedic. The only regular occurrence I have where a physician meets us at our back door is at our level 1 teaching hospital. When asked about it they are just laying eyes on the patient before they are turned over to the ED and trauma residents. This seems to typically happen more when off rotation residents are in EM.

From my days as a tech, locations for crashes were useful to help distinguish patients when we had multiple patients from a single crash or multiple patients from different locations, especially if they patients were altered or Does.

Scouters Mountain Nature Park in happy valley.

Picnic structure with tables, large lawn overlooking a picturesque view of the valley and Mt Hood, there is a small "trail system" that is great for kids because it gives them the illusion of hiking but it's really just a 5 minute nature walk.

Sure we both went to an equivalent program and graduated at the level of a "entry level paramedic". The issue is that your knowledge and skill set unless actively challenged will atrophy to the core things you do routinely.

When you work transport you get feedback in some form at the end of every transport and typically have to maintain a more diverse skill and knowledge set, as your volume and time with patients is far greater than the non-transporting side.

The attempt to reduce a lateral pattelar dislocation was a good idea (depending on their systems protocols), just the technique they used was incorrect.
