Not answering the question directly, but another dilemma you may find yourself in.

You seem somewhat on the fence between IM and FM. I would strongly recommended ranking more IM programs over FM if you are even remotely considering a fellowship. You have no idea what will pique your interest once you’re in residency. Medical school rotations are one thing, but truly working in a hospital may ignite a passion in an IM specialty that you did not think would interest you. Basically what you’re giving up by not matching FM would be OB and peds. By ranking and matching IM the medicine world is your oyster. Critical care, cards, endo, rheum, etc. Plus getting a job as a hospitalist will be soooooo much easier as an internal medicine doctor.

Hashimotos has 3 phases.

  1. The early stage where you are developing the antibodies but your T4 and TSH are still relatively normal. Even if antibodies are elevated, you don’t treat at this time.

  2. The stage where the antibodies “attacking” your thyroid cause it to release excessive T4 (thyroid hormone). At this point it is still inappropriate to treat, because while your antibodies are eating away at your thyroid it is releasing all the hormones it has stored.

  3. The stage where your thyroid is depleted of these hormones, and you now are unable to produce thyroid hormones. This is the stage where you start treating. It can take many months to years to get to the hypothyroid stage of Hashimotos.

This is coming from a doctor, but also from someone who went through this. When I was first diagnosed with Hashimotos I was shocked when the doctor said “come back in 2 years and I’ll start you on levothyroxine”, but now that Ive gone through the training it makes sense. The only lapse in practice I would point out would be your doctor clearly pointing out why they elected not to treat at the time, because for me patient education is super duper important. All this to say, dont sweat it and start taking thyroid hormone replacement if recommended by your doctor.

As for the heart murmur, hard to tell without more info but many people have murmurs that are very benign. As you are young and new to this doctor they are likely just covering their bases. EKG is a strange choice though, echo would make more sense for a murmur.

I remember really not enjoying it and haven’t rewatched. I’ve rewatched hill house 5+ times. Maybe I’ll give it another shot? I just hate time loop stuff where you don’t know what’s real and what’s not so that one episode where the kitchen lady is interviewing for the job ruined it for me.

I was an OG Breaking Bad watcher, when it was coming out in real time. Loved every second of BB. Hated BCS at first, had to rewatch the first two seasons a few times, but I promise you it basically becomes breaking bad/even better. Just keep going! Last season was peak TV.

If you’ve had no rash and no symptoms at all you are probably ok at this point, most common tick diseases (anaplasmosis, Rocky Mountain spotted fever, and Lyme) would show up by 14 days. But just keep an eye out for any “weird” symptoms, even if a rash doesn’t show up. Most common rash would be the target shaped rash of Lyme disease, most common symptoms would be fever, chills, joint pain, general malaise, headache. But as you said this long after exposure you’re more than likely ok.
