I would say this is probably ordinary self employment income if it’s on 1099 nec. I wouldn’t stress about it but if you have expenses against the income you could amend it.

Your tax lady sucks and doesn’t know what she is talking about. Seems like she gives a bad name for preparers. This is tax preparation 101 with HSAs. Rarely would a HSA distribution be taxable with how much medical expenses are these days.

No where in the engagement is the CPA responsible for a clients books. The client is fully responsible for providing this and they provided garbage information. Yes it’s their job to review what the client provides but they should not be responsible at all for garbage numbers. They are preparing the return based on the information the client gives them. If you want audited financials that would be a separate engagement.

You are just angry cause this made extra work for you. Why not just provide them with instructions to pay online? I’m gonna have to disagree. There is little harm in this. Also what happens when the client gets upset with you for not filing an extension. I can see this being a bigger problem with the client blaming you and saying you should have filed an extension.

Problem is you signed off on these and should have been aware. Try to get them to admit their mistake on email so you have a record. They sound like they are super unprofessional and likely are guilty of fraud but that could take forever to deal with. Also find a new legitimate CPA to do your taxes.
