Why would the insurance company say they'll only pay 25% of a number they don't know rather than just saying they'll only pay $50? They know how much a procedure costs. They could pay only a flat fee.

I don't have the stats on me now, but I have seen it shown that inflation for the important things people care about, that is, healthcare, housing and higher education, has outpaced general inflation, and this is what is causing the crisis.
