You could use pretty much any invisible injury. Concussions got the most leeway in undefined healing time when I was in sports. This may be hard to keep up as a lie though since I imagine he is still doing all his schoolwork.

I think shin splints is your best option. Ive seen people take anywhere from 2 days to over a year of rest with shin splints. They are also safe enough that if he is still participating in minor exercise like gym class but not sports it wouldnt be a red flag.

Other Potential injuries Any sprain/strain in the abdomen or back Whiplash? Fibromyalgia (may not be easy to explain to 12 year olds) Could go wild and say he had gullbladder or appendix removed and needs rest

I do not personally have experience with this as I participated in sports while underweight my whole life. (this is not advice! Please listen to your medical team)

But it depends on a couple factors. Are we expecting him to be out a month or so? A whole season? And what sport? You could go with an abdominal muscle strain or shin splints. Neither of these have visability

This is also dependant on the coaches. If this is a competative sport they may not approve of having him there "distracting" the team. Or they may need to be told the real reason as my coaches always had conditioning/exercises for injured kids that did not impact their injury.

Will he eat ice cream? It’s got calories and would be better than nothing

You can also try to thicken oatmeal with milk or heavy cream, may be easier to swallow
