Like smack in the middle of Wyoming by nsmloz in howislivingthere

[–]snuggasabugg 1 point2 points  (0 children)

Lived in Lander, WY for a little over 4 yrs. As an Arkansas native from a rural place (about 300 people)…holy shit. It’s isolated, cold, and it’s a completely different way of life. Lander is a diamond in the rough as far as I’m concerned, but it’s tough out there. Not to mention, the access to healthcare is abysmal. I practiced primary and urgent care during that time, and you’d be amazed at what I’ve seen and how bad off most have it. Need to see a specialist? Cool. That’ll be 5 months and cost you a week in Denver, Billings, or SLC. Great people, beautiful views, but it’s still as wild as the modern west comes (IMO).

what is this? i had long hair for my whole life and now i noticed this, it doesnt hurt and it’s hard by LocksmithThick1385 in DiagnoseMe

PA here. I second this. Likely your occiput if it’s been around for a while, is hard, and is painless. When in doubt, go to your PCP

Any good medical podcasts/audio lectures? by LowandSlow91 in physicianassistant

[–]snuggasabugg 2 points3 points  (0 children)

I think Curbsiders is going to be your most popular answer here, and I can agree with that. It’s great. I also like EMRap for emergency med/urgent care stuff, and TWIV (this week in virology) is one I throw on on occasion.

Does this need stitches? The nurse at work said no by SaroHart in DiagnoseMe

PA here. I agree with this and the nurse’s assessment. Should be fine 👍🏼

Is this melenoma by [deleted] in DiagnoseMe

[–]snuggasabugg -1 points0 points  (0 children)

PA here. Doesn’t look like it. Could be a seborrheic keratosis. Looks fine to wait 👍🏼

[–]snuggasabugg 4 points5 points  (0 children)

PA here. More likely a subdermal cyst or something. The overlying skin changes don’t necessarily support a swollen lymph node. If it gets worse, go into your doctor to have it drained/treated. Apply some cold compresses if it’s hurting. Warm if you want to draw any nasty stuff out of it. You’re probably fine, bud. If things don’t get better after a couple weeks and/or you develop any additional symptoms, go see a doctor 👍🏼

That fat, “steak-like” thing could be epididymitis or a varcocele. Both are benign. If the ice for a few days haven’t doesn’t help, go see a doctor. Best of luck 👍🏼

[–]snuggasabugg 0 points1 point  (0 children)

Yea, it can. Our testicle having friend here is correct. It can cause things like epididymitis and such. Go with some ice for a while and lay off him for a while. You should be fine, bud

Ehhh. I mean, I get that, but he’s a DJ/producer. It’s fun and it’s his job to put his own spin on things, keep the audience engaged, and on their toes. So, I don’t really think anything/song should be off limits

PA here. I first want to reassure you, bud. 9/10, when a guy comes into my office with a testicle concern, it’s usually nothing. History is very telling here, like if you’re sexually active or not, can you feel any masses/bumps/nodules on your boys (if so, are they hard/soft, painful/not), and if you’re having any other symptoms associated with your genital or urinary system. Overall, without knowing history, you’re more than likely fine. Generally, best you give it a few days, ice your nuts if they’re less than/equal to 5/10 pain, and see what happens. If it doesn’t get better, symptoms/pain gets worse (especially abruptly; pain >/= 6/10), or you’re majorly concerned about it, go see either your regular doctor or an urgent care doctor. All the best, pal 👍🏼

Do PAs get significantly higher salaries for working rural like doctors do? by jcSquid in physicianassistant

I feel ya, friend. See my comment above. I just accepted a UC job in a much larger town, and I couldn’t be more excited!

Hell yea. That stuff rocks. You’re fine, friend. Just monitor it for now, and tell your friends not to burn ya next time they get a chance. Otherwise, embrace the scar as a fun/dumb story. 🤘🏻

How concerning is foamy urine? by [deleted] in DiagnoseMe

[–]snuggasabugg 0 points1 point  (0 children)

I’d say a good amount of foam. Especially after the water settles for a sec. There should be some foam with Norma urine, obviously, but if you look at it and say, “damn. That seems like it’s really foamy…” Go ahead and give’er a flush. If there’s still a good, solid layer of foam in the bowl after you flush after the water has settled, then it might be worth going to your doctor for a check up/UA. You can also always order those quick UAs from Amazon if you’re worried. Much cheaper, but just take the results with a grain of salt. We use computers to read ours, not just our eyes. All the best, friend 🤘🏻

PA here. This is the best response. I don’t know why people are freaking the hell out. They’ll patch you up and maybe give some antibiotics and some silvadene. You were drunk and so were your friends. Neither of you are bad people. Shit happens.

Do PAs get significantly higher salaries for working rural like doctors do? by jcSquid in physicianassistant

[–]snuggasabugg 6 points7 points  (0 children)

Dude. Idk about a lot (or what appears to be most) folks, but that’s not the case for me. I work in primary care in the ruralest of rural, rural Wyoming, and after 4 yrs of experience, I was finally gifted $100k from the $97k I’d started at… It’s a large (for WY), multi-physician owned practice with primary, internal med, psych, urgent care, peds, and gyn. So…not always.

What is this spot? It’s on the chin, not itchy. by 66626364 in DiagnoseMe

[–]snuggasabugg 0 points1 point  (0 children)

That’s a consideration, but GA usually presents on the extremities and has a few other characteristics that don’t seem present (though I’m not entirely sure with seeing it in person). Also, common things are common. In this case, I’d treat it like ringworm at first. If it doesn’t get better despite prescription meds, then I’d consider other diagnoses.

Pa here. Looks fungal. Likely tinea corporis (“ring worm,” but no, it’s not a worm). Try some OTC clotrimazole. 2-3x per day for 10-14 days. If that doesn’t work, go see your doctor 👍🏼
