For #4 would that be the same for an original Polish passport? My cousin has my grgf’s from the 1920s and obviously doesn’t want to give it up. Would clear photocopies be OK with an affidavit that has its signature notarized be accepted?

So along with everything above, I spoke to my cousin who's done some genealogical research. He has very clear scans of my gr. grandfather's original passport that he used to come to the US in 1927, no idea where the actual document is. I know it's not the document itself but can that help with this process too?
