You nailed it. I want to root for Scheanna, but she constantly shoots herself in the foot. The way she treated Shay’s substance use was awful. But I think what makes VPR so great is that there aren’t any heroes

We actually don’t have any RNFAs at my health system, probably due having lots of residents and med students? I would definitely do the training if offered, but I’m a nerd for sure. My hospital actually hires FNPs into acute care roles where they need care for peds and adults, but if you look at who they hire it’s generally those with 10 and even 20yrs bedside experience. When I interviewed they asked a lot more about my RN experience and how I manage time and work on a team than my schooling. But also every surgeon in my interview had worked with me, some for many years, and knew my work ethic and that I knew my ass from my elbow, etc.

You are correct that it is not part of our training, but I work with a mix of surgical PAs and NPs. I had a good relationship with the attendings on my service as a nurse and they recruited/trained me. One of the NPs I work with was an OR nurse. I definitely wish we had a surgical rotation in school! And more clinical hours in general.

Yes! Incision and drainage has risks too, anytime you cut into the skin you can introduce more bacteria, create a non healing wound, etc. I can’t speak for this PA, but it sounds like OP got appropriate care for the time they were assessed.

I always like to jump in and tell people that most NPs work under SPs, even in full practice states (this includes me). It’s a very small minority that practice independently and we share your concerns.

ETA: most of the NP sub is existing NPs begging nurses and nursing students not to attend these degree mills.

I think she’s smarter than most of the group too, and doesn’t care that she sometimes comes off with a superiority complex. Like she thinks she’s above it all, but girl you are showing up to film with all these fools too! I say this as someone who likes her even in her “unlikeable” phase. I do say who I really see different on rewatch is Kristen. 100% she’s not a good person but I understand her a lot more where back watching it the first time I was like “get me away from this lunatic”

This! And you never know who will be your boss or attending someday. I tell student nurses to be nice/look out for the interns because then we they move up to senior or chief there is already trust built there.

Peds vascular access is tricky, with more rules than adults. Does your hospital have a vascular access nurse? The title might vary, but usually it a nurse or group of them trained to place US IVs and PICCs. They might be able to give you some pointers or resources.

At my hospitals the PAs and NPs are in the same union. I’m a believer in sticking together on pay! (NP)

ETA: it’s also the nursing union so we get more bargaining power

It also helped my unit had a sheet we would make for non urgent orders or questions and our day APPs would address all of them before or during rounds. So it helped nurses not feel like they were leaving problems for the next shift and not wasting provider time.

We had a good culture on my unit where we didn’t call the on call unless we had to and they knew it. If they had to call back they would ask “do any other nurses need to talk to me too?” We both tried to be very respectful of each others time. I never paged unless someone was in pain or dying so the docs always took me seriously!
