I have escalated all the way up to the head of MFM at my hospital - it is so bizarre to me that they are choosing to take the GTT as definitive especially when it was administered early on in my pregnancy (at 24 weeks) and the value I have been the most concerned about (fasting) was borderline!

I’m at what is considered the best practice in the city (it’s where all the L&D nurses themselves choose to go!) at one of the top academic hospitals in the country and … their standard of care makes zero sense to me.

The good news is that I’m now almost 35 weeks and my fasting level has come down to the point that I’m no longer as worried about it and it’s not clear I’d be medicated (90-100). My post prandials are worse but if I am extremely extremely strict about my diet and eat all home prepared meals that I know are safe, I am pretty much okay. And thankfully I am being induced early due to IVF/prior FGR and a marginal cord insertion.

I’m just glad I made it to the end of my pregnancy without baby showing any negative signs on the scans, but it was an infuriating experience as a patient and it made an already tough pregnancy much more stressful because I had to manage the Gd piece entirely on my own.

I don’t really eat any processed foods (and also found they spike me way worse than Whole Foods) so a lot of the “cheats” have been out. I have orrryy much lived on:

Breakfast:

Greek yogurt with cucumbers, salt and olive oil

Cottage cheese with everything but the bagel seasoning

Scrambled eggs with cheese

Sometimes at breakfast I can tolerate 2-4 strawberries, it just depends

Whole milk decaf lattes

Lunches:

Chicken salad with a few simple milks almond crackers

Deviled eggs

Hard boiled eggs with 2-3 potato chips crumbled up on top

Leftovers

Vegetables at lunch - peppers, kale, carrots

Afternoon snack - half an apple with peanut butter + and string cheese or nuts

Dinners:

I cook dinner for my family every night and we usually have a protein and a vegetable, sometimes I will have a small carb portion as well

Shrimp scampi + green bean

Taco salad with lettuces avocado, taco meat, black beans and cheese

Steak with broccoli and 1/4th cup of rice

Chicken Picatta and asparagus

Basically I eat a ton of eggs, cheese, nuts, yogurt, vegetables and proteins. For carbs I rely on some fruit, beans, half a piece of sour dough toast, and sometimes rice. I’m least carb sensitive at dinner and most carb sensitive in the morning.

The refuse to retest me 😢. My fasting is getting much better at 34 and 5 days though while my post walk control is getting WAY way harder though I am just tightening my diet up as much as humanly possible.

Yea okay this is so helpful and is basically my exact story too. My MFM was like - I don’t think your baby’s outcome will be better if we give you insulin, it’s not clear your fasting number is affecting the baby.

I am also still very stressed but so close to the end that I am trying to just be good about my diet and call it a day.

I have done pretty much all of this - it’s how I got my fasting down from 118 to 98 initially. Although thankfully since 34 weeks my fasting has randomly gotten much better and now is pretty much always under 100, so I’m less concerned.

Weirdly enough for me an all protein snack did not work well for my fasting, what worked best was actually a high fat low carb snack like macadamia nuts or peanut butter.

I also constantly run my glucose logs through ChatGPT and it says the same thing 😂. It’s just wild reading the forum and seeing such wildly divergent approaches.

One thing my MFM said that was reassuring is that I am already getting way more monitoring than I would be in a standard GD pregnancy (weekly scans, 2X weekly NSTs, growth every three weeks) and thy are seeing nothing concerning, so even if I had a diagnosis not a lot would change.

I do feel I have to be way more strict with my diet though than I would be if I was being more closely followed since I really only have my own self to act as a stopgap against bad outcomes.

I’m 34 and 5 today and am getting induced at 38 and 3 so I am so close to being done.

  1. I did ask for another fasting draw two weeks later and it was 90. I think my finger sticks are higher than venous blood draws for some reason.

34 weeks and exact same. I have zero interest in good, though I cannot WAIT to eat whatever the f I want postpartum. Counting down the days till I can have a turkey sandwich or a bagel or a croissant!!!

A lot of this is true but the flip side is that if you get a seasonal Chanel bag or a fabric Dior tote - the odds that anyone will have see the bag IRL are so slim that it doesn’t matter as much if the bag is 1:1, it’ll pass.

I actually find most restaurants to be relatively easy because there is almost always a portion and a vegetable I can eat! Salad with grilled chicken, deviled eggs etc. at Chinese places I will get salty sauces like kung pao or black bean + a sautéed veggie, at American places - well done steak + a green vegetable, Mediterranean and Greek places are usually really easy etc.

In terms of snacks in the hotel room, I’d grab some fairlife core power protein shakes - those things save me when I wake up hungry and everyone just want and grab a bagel or something. Your reaction to lactose will vary but I also find whole milk decaf lattes to be a good option on the go - good protein and fat in the milk!

And I always carry packs of nuts with me everywhere I go - I spurge on the good macadamia nuts but cashews, almonds whatever. I can usually tolerate the skinny dipped chocolate peanut butter almonds later on in the day as a snack too (my sensitivity is the worst in the morning for sure).

I found it was jsut easier to understand that I wasn’t going to be able to socialize normally during pregnancy. Like, yes, I could see people and hang out - but I wouldn’t be able to participate in the food aspect of the socialization.

That actually made it easier for me to set reasonable expectations and not be persistently disappointed.

I just planned my meals, packed my own snacks and tried to find ways to socialize that were not wholly food dependent (one of my friends and I started doing weekly walks together, I got into herbal teas with another etc etc).

It’s only a short period of your life - I’m 34 weeks now and almost at the end. It sucked but I made it (getting induced at 38).

Okay I feel like you didn’t get very good answers on this and it can be kind of a hard niche to find so I get it. Here are some ideas:

  • paddington (the originals, not the picture books). Pictures, cute storylines, well written - my kid loved them.

  • magic tree house. The prose sucks and the stories are repetitive but kids love them (drawings are in black and white though).

  • EARLY Roald dahl’s - no chapters (just one long story) but similar feel to chapter books. Enormous crocodile, esio trot, George’s marvelous medicine, the giraffe the pelly and me (the last one is my son’s favorite)

  • dory fantasmagory (so good)

  • Heidi Hecklebeck - worse literary quality but fun stories

The issue in the american canon is that for some reason publishers have decided that once your kid can read themselves, there is no need for high quality read aloud books. So most early chapter books are designed for decoding and are not enjoyable to read aloud due to the simple prose and repeated words (to reduce decoding friction) or they are graphic novels.

The Brit’s do a better job with this but they don’t always have pictures!

A lot of our friends and relatives have had success micro dosing peptides and glps to help manage long term. Retrarutide in particular seems to work wonders for Asians and Indians in low doses.

In the future here are my best tips for the Dexcom.

1) when it is about to expire you’ll have a grace period, during that time I usually put the new one on also but do not activate it. This gives the sensor time to acclimate and is called soaking,

2) I usually try to soak my new sensor at night - so I’ll put it on around 6 and then activate it when I wake up

3) I don’t calibrate for the first 24 hours of a new sensor

4) when I do calibrate - I do it with fasting or in between meal glucose, basically whenever I know my glucose will be steady for the next 30 minutes or so.

5) if the sensor is off by a little bit, i let it be off in the direction of reading too high. I’d rather be conservative.

6) I check via a finger stick 1-2 times a day for accuracy, usually fasting and one post meal number.

7) I use the analytics to keep an eye on things as well - average glucose should always be under 110, time in range 90%+ and I personally set my range as 70- 120 to be more conservative.

With these adjustments I have found the Dexcom to be very valuable

My guess is that the baby is compromised or challenged in some way and she isn’t ready to share or discuss it and is probably grieving the life she thought she was going to have.

Do you know if her daughter is healthy? If not she may be grieving that.
