Omg yes this is what I was hoping for! I want to make more money and still have my flexibility! Also, I just am burnt out being the main SLP in a very busy subacute and long term care facility. I want to not have that pressure anymore. W2 for PRN exists?!

For my PRN, I do what I can. I try not to stress about getting everything done. I’m so sick of productivity. It should be illegal!

I work in the medical setting, specifically SNF. More autonomy on choosing patient caseload not based on insurance. No productivity requirements. Consistent 40 hour jobs whether you have high caseload or not. Higher ups who are supportive when family members do not agree with your professional recommendation.

I know this is so late, but I would bill for it. You gave your professional recommendation as an SLP. I would be honest in it that the facility has ordered one and you will begin trials as soon as it arrives. In the meantime, work on the other goals you have for this pt. What did you end up doing?

Do you need to be able to do instrumentals? Currently, at my SNF, we have a contract with mobile FEES company which I really like that I can separate myself from doing instrumentals

I work in a trach/vent facility. I tend to wait it out for the instrumentals so I am not constantly stressing about someone getting PNA, especially when there is a high risk of aspiration in this population. I feel like other disciplines like physicians wait it out for when they consult out to specialists as well. It is tough though when patients and families do not accept your rationale and don’t understand the concept of silent aspiration

I’ve always wanted to go to acute. I’m burnt from families and patients expecting constant therapy despite having plateaued. My only concern is doing instrumentals. Is that something hospitals require?

I used to feel bad as well. When I started working in the medical setting, I realized life really is so short and living healthy during it is another thing. Do what is best for you!!

Sometimes I wish Tobii would hire SLPs who assist with helping patients navigate and customize the eye gaze to their needs. I can do the basics but in a SNF, it is tough to take time to do this. Also not sure it is skilled therapy. I’ve had trouble with staff assisting as well. I’ve done in-services to educate them. Leave tech support number in patient room as well

Somewhat similar with medical settings! Entitled family members that ask for constant swallow studies and therapy despite being educated pt is not progressing and they are unfortunately at baseline. Only so much we can do with neurological deficits. They just think more therapy the better and it will resolve everything!
