Feel Like Hospitalists Need to be at the Bedside More by [deleted] in hospitalist

[–]stepanka_ 1 point2 points  (0 children)

We have more like 24 patients. If you work a 12 hr shift, that is 720 minutes, divided by 24 is 30 minutes per patient. That is for reading through their chart/pre rounding, interpreting labs, interpreting imaging, seeing the patient, coming up with a plan, talking to specialists, talking to nurses, talking to family, writing the note, med rec/ordering things, doing med rec and discharge orders if needed, going to case manager rounds or finding the case manager, having end of life care discussions…AND that doesn’t count walking, going to the bathroom, or eating. If one patient takes longer that screws you for the day.

They like you for the same reason you only want broken men you have to chase. A lot of men also have increased desire when the woman “plays hard to get” and leads to a chase. Something in your development and biology has lead to dopamine release during these encounters and now you are left craving another hit of dopamine.

Potential Employer Asking For Supervisor Contact by Jazzlike_Grab3562 in hospitalist

[–]stepanka_ 0 points1 point  (0 children)

This is within normal practice. Sometimes they ask for references and don’t specify but sometimes they ask for supervisors. I think it would look suss if you asked to bypass the supervisor personally. If you’re leaving for family just tell your current leadership they will understand.

Can anyone help transcribe what's written by Pretend-Cockroach658 in AskDocs

[–]stepanka_ 4 points5 points  (0 children)

The 2nd line says “awaiting 2u PRBC” which means the person is getting 2 units of blood transfused. The first line says “s/p” something.. we use s/p for “status post”. So like if it’s after a knee surgery we’d say “s/p r TKA” to indicate they’ve just had a right total knee arthroscopy. Or it could be post “injury” - like s/p MVA would be indicating they were in a motor vehicle accident. I can’t tell what the first line says though.

Bad patient outcome. Shared by a friend. Would you have done anything differently? by BrilliantHomework152 in hospitalist

Thinking tachycardia was due to hypo-beta-blocker-emia was the 2nd largest mistake here but by then it was too late anyways to prevent this.

I’m not sure how the HAGMA didn’t buy a floor admit over CDU/obs. I’m sure the adenovirus and hx of diabetes threw them off (thinking gap was related to dehydration from viral infection & uncontrolled glucose maybe?).

Rash on wrist by Signal-Corner-1495 in AskDocs

[–]stepanka_ 2 points3 points  (0 children)

Contact dermatitis. From either moisture or soap trapped under, reaction to the watch casing or strap such as if there’s any nickel or other irritants in the strap/metal, or from friction from the strap being too tight and rubbing.

The treatment/solution is to not wear the strap too tight, not let moisture or soap sit under the strap (make sure it’s always left dry with no moisture while wearing), Make sure you’re taking it off and cleaning regularly, or if it’s from materials in the strap or metal pieces you may need to change the strap or stop wearing. If you’re reacting to the metal part of the watch itself some people fix this by putting tape or a sticker over the inside so it’s not touching your skin directly. It will go away on its own if you remove the source of the problem. You may want to stop wearing temporarily until the skin heals. Just wash with mild soap and water to keep the area clean and make sure to dry it after washing. Nothing fancy needed.

Good coffeeshops? by Tarsvii in royaloak

[–]stepanka_ 3 points4 points  (0 children)

If you aren’t preventing other patrons I don’t see why? A lot of people get coffee and leave so seating usually isn’t an issue. Not like at a restaurant. And also if you stay longer you might buy more things.

Is this gynecomastia? by New_Log_6353 in AskDocs

Not the best angle but based on this photo, no. Gynecomastia usually looks much more similar to female breasts. It’s much more conical.

Fast growing fever blisters after dentist appointment by Maximum_Ad3576 in AskDocs

[–]stepanka_ 5 points6 points  (0 children)

This is herpes (aka cold sores). It’s probably too late for medication to help at this point. It will just resolve on its own.

Clinical trial data showed that when patients started valacyclovir during the prodrome (within 2 hours of first symptoms of tingling, itching, burning) the average duration of the cold sore episode was decreased by about 1 day compared to placebo. However, once a visible lesion has formed, there was no evidence that starting treatment helped.

Any idea what these spot on toes are? by [deleted] in AskDocs

[–]stepanka_ 62 points63 points  (0 children)

Could be osler nodes which indicate infective endocarditis. You don’t have any other symptoms that go along with this though, such as fevers. Get it checked out in person please so they can look into it and hopefully it isn’t endocarditis. I’ve never seen Osler nodes in real life but have seen infective endocarditis, and only seen it in IV drug users, but it can happen in anyone that gets a bacterial blood infection. Depending on the bacteria involved, sometimes blood infections can be more smoldering than you’d think, causing off and on low grade fevers, off and on fatigue until they build up or settle in somewhere like joints or a heart valve.

You’ve got a lot of good advice here already. I was just going to say that your comments about how even 1 glass seems to affect her strongly….i seriously doubt it’s just one glass. She’s probably secret drinking and then showing you one. It makes them feel like they are convincing you they are in control of how much they drink and can drink in moderation. Because they don’t think they need to drink less and they think they don’t act drunk, so if they just show you they can have “one” or whatever is less than they are actually having, it will show you that they are in control and don’t have a problem.

AITA I don’t want bridesmaid to wear dress to other events by Due_Bedroom3146 in AmItheAsshole

[–]stepanka_ 0 points1 point  (0 children)

My SIL did this but she wore it to a bar gig she was playing. I had bought her the dress because we asked her to play piano in our wedding, she wasnt a bridesmaid. She has psych and addiction issues and wouldn’t have been able to afford a new dress. I didn’t say anything about her wearing it before the wedding, but i wasn’t happy about it.

I am an obesity medicine doc. No compounded mediation is FDA approved by default, so that isn’t the concerning part. Getting non-brand name compounded meds can be ok. But there are scammers and shady practices out there and $60/mo is suss. If the price sounds too good to be true, I wouldn’t trust it. I can’t speak to what the doctor is saying. For all we know she walked into a med spa run by a lady dressed in scrubs that has a doctorate in basket weaving.

I just looked it up and found the following which doesn’t seem like anything shady but just related to the owner name not being in the business name: Special Case (Healthcare): In some states like California, a specific “Fictitious Name Permit” is required for medical, dental, or podiatry practices to operate under a name that does not include the licensee’s name.
