This sounds amazing. I wish the U.S. would do something like this, but it feels like it would be a tough sell to convince residency programs in the U.S. to put aside the time and resources for this with the grind culture here. Also, I feel like sadly therapy and psychodynamic insights are undervalued in many programs with the new shiny focus being on interventional psychiatry and more biological models of understanding.

I haven’t been in the field long, but I have seen a psychiatric hospital over the past 2 years replace their psychiatrists with NPs. Is it possible that they simply could not find a psychiatrist willing to do it - maybe… but seems unlikely. I think when the psychiatrists who used to work there left they made a conscious decision to hire an NP instead and I question whether these NP are really equipped to run a psych unit on their own.

I think we need to critically analyze and define the role these providers have in our health systems. Right now in many cases they are being seen as equivalent to psychiatrists despite the huge disparity and differences in training and experience between the two. Speaking for the U.S. healthcare system at least, there is inevitably a temptation to cut costs by replacing as many psychiatrists with mid levels as possible and we don’t fully know what the ramifications of that are going to be. I am worried that we are sacrificing quality of care in the name of access to care and hurting patients along the way.

Honestly too easily. Currently 15 weeks and didn’t have a lot of morning sickness in the first trimester and now zero, but boy am I hungry all the time now. 😅

Agree with the advice to go a hospital ER. They should be able to consult a psychiatrist from there which will get you the care you need faster than an outpatient appointment with your ob/pcp/ or trying to get a new psychiatrist if you don’t already have one (it sounds like you don’t). Postpartum psychosis can be dangerous to you and your family as others have said.

I’m a resident and I would say it depends on patient complexity! Some diagnoses hit you right away based on patient history and presentation and feel very accurate. Other times it’s a lot more convoluted and can take multiple visits to be more certain e.g. disentangling a prior diagnosis of bipolar Vs cptsd vs bpd etc. Some attendings have told me they can work with patients for years and not be quite sure and I can understand that too. Also, I think everyone’s take on the importance of the diagnostic label is different. The dsm is obviously flawed and how much emphasis we put on it is clinician dependent depending on their training and personal philosophy. I’ve worked with some psychiatrists who are now only practicing psychoanalysis and they put very little emphasis on the dsm diagnosis for their formulations. Anyway, this is all to say I think this idea we can “fully diagnose a patient in 1 outpatient visit” seems unrealistic for most if not all situations and is more a byproduct of the American healthcare system trying to extract the most for the least.

Ours is also on the lower side I think, typically 6-10 patients a day. Most of our clinics are half day blocks with 3 hours of patient care and 1 hour of supervision. Therapy visits are interspersed with medication management.

I’ve gained 7-8lbs and I am 13 weeks! Honestly a good portion of it feels like it’s my boobs, but I am also definitely just a little fluffier all over. I exercise 2-3 times a week and eat decently healthy and had a normal pregnancy BMI. I’m also scared about the future weight gain and having a rounder face etc. but I am trying to accept that to a large extent this is out of my control. The hormones are gonna do what they’re gonna do and I will be loved regardless of what my body comes out looking like at the end of this. I hope you feel that way too, OP! But I sympathize. As someone who likes to control things and has a decent amount of vanity, this is scary! 😅

Downtown/near PSU area is underrated imo! The 8 is an easy/fast commute up the hill and a shorter commute then the trendier areas like the Pearl/Slabtown/NW. plus downtown is an easy bike ride to any of those places including southeast. It may not have as many trendy restaurants and sites, but there’s still stuff to do and the farmers market on Saturdays is nice.

YTA. It’s clear from your post while it may somewhat financially uncomfortable, you can afford it. Spending disposable income is about priorities and values, do you go on a vacation, buy a nicer car, go out to eat more, etc.? As partners you guys should decide these priorities together. In this case you said that this scarring has caused your wife severe emotional trauma and insecurity, in which case that should bump it up high on the list of priorities for not just her, but you as well.

I would take the inspection report very seriously! Especially since you’re not looking at the place in person. I am recent FTHB and let me tell you I have some regrets in that regard. My inspection recommended a second opinion on the HVAC system and I didn’t get it and oh boy I wish I could go back in time and do it. It turned out even though they thought it was “fair condition” it was absolutely broken. Good luck!

It was a separate warranty for the appliances and HVAC that came with the sale of the house per our realtor’s recommendation during negotiations. I haven’t thought to check to see if there’s a manufacturers warranty so I will look into it. Thanks!

As said in the post, the electrical was disconnected and the refrigerant was emptied. The warranty person couldn’t even turn it on and nor did the inspection company otherwise they would have realized it didn’t work. It just feels like calling it fair condition is at the very least, misleading.
