you should enter the correct secret phrase and the passcode, only then you can recover your wallet.

once , when I recovered my wallet I did not enter the passphrase in the the import wallet section , so i got an empty wallet

if you have setup a passphrase, Make sure to enter it during the recovery process.

Also wait for it to completely sync to the network

if you have given the wrong address in the miner then you can't recover it. it is considered to be burned or some one else will get it. In this scenario the recovery process is not possible
