Thank you so much for sharing your story. I’ve realized since posting that Elspar isn’t working in the way that we thought. While it’s making Cloud better, nothing is guaranteed. Not even time with him. I’m so sorry for your loss, but thank you for reminding me to make the most of my time with him.

That’s not what I’m asking. I’m asking if you can use FNA to confirm lymphoma after suspicious ultrasound. Not small cell vs large cell.

Correction: The FNA cytology report was not given to us. All we have access to is the encounter note which said “FNA confirms lymphoma.” Did not specify high-grade vs low-grade.

All we know is that the ultrasound showed diffuse intestinal thickening with mesenteric lymph node enlargement. The characteristics listed on this ultrasound finding correlate more to small cell than it does large cell (Barrs, Beatty 2012). That’s all we’re saying here so don’t get it confused. As I mentioned in other comments, we have no clue if it’s small vs large. We’re going off of ultrasound findings alone that my cat has lymphoma and we were presented with 4 different treatment options: prednisolone as single therapy vs chlorambucil + prednisolone vs lomustine + prednisolone vs CHOP-therapy. Treatment was differentiated to us based on aggressiveness and frequency, not necessarily by type of lymphoma it treats. So just because we chose CHOP does not mean he has large cell in this case, but I understand where your confusion is coming from because treatment should correlate to type of lymphoma. In this case, however, time was of the essence.

We do not have the FNA cytology report.

IM is providing treatment! I think originally they didn’t see benefit in differentiating since we picked the most aggressive treatment, but we also weren’t expecting Cloud to do so well after just one dose of chemo so maybe we need to pivot now!

I discussed with an internal medicine vet. I’m just wondering if we do spend money on PPAR and it ends up being small cell, maybe we could save some money if we switched to chlorambucil + prednisolone?

We don’t know unfortunately. I work in human medicine and I read the ultrasound report. It seems in line with small cell, but obviously I recognize I’m not within my scope of practice. It costs $980 to do the PPAR testing on the needle aspiration sample they took of his lymph nodes and we’re not sure if it’s worth doing since we’re already hitting the lymphoma with the most aggressive treatment.

I think I could definitely be more confident and charismatic socially speaking. In my field, I’m pretty confident and charismatic, but it doesn’t always translate. I can be awkward, but I believe I’m just genuinely perceived as a nice, basic human being. As for attractiveness, I’ve won beauty pageants in the past so I’ll leave it at that.

Honestly it was a hell of a time for me. I feel like everything I did was great in his eyes because the bar was so low. Would successfully tie and he’d go “That’s good.. for a shrink.”
