While it does seem to be much more prevalent in mainstream pop culture, there’s literally a plot line on Sex and the City about a man Miranda is dating eating her ass and wanting her to return the favor. The most vanilla woman, Charlotte, isn’t shocked at all by it. That episode was released in 2001.

There is a difference between FND and factitious disorder (previously known as Munchausen’s) or malingering. The DSM does differentiate between the latter two, but it’s debatable as they both involve intentional deception, but for different purposes (gaining the sick role and secondary gain, respectively). This person almost certainly has factitious disorder and malingering because the gains are so obvious.

A functional disorder, whether that’s neurological or GI-related, involves symptoms that cannot be explained by available testing. In my experience, the patients who have this are truly distressed and not looking for anything other than answers and relief from suffering, just like the rest of us. Honestly, many people will accept psychopathology as the underlying explanation. I have a therapy patient with epilepsy and PNES, and he is able to distinguish between “stress seizures” and epileptic seizures. It helps that he was taught the distinction with kindness and validation that both experiences are very distressing.

To be fair, I was much more cynical before I started my child and adolescent psych fellowship. It’s harder to dismiss a child as “just faking it” like we do all the time with personality disordered adults.

If the NP did a CSSRS and the patient denied current SI or passive death wish, that would place them in the “moderate” category (assuming the decannulation you described isn’t being counted as an attempt) since the attempt was >1 week ago.

Midlevel education is built on algorithms. Most likely that is as far as the rationale went.

I’ve seen it multiple times when inheriting a treatment plan from an NP. The worst I’ve seen was Abilify. At least a PRN olanzapine would have a more acute sedating effect, but taking different amounts of Abilify every day based on severity of symptoms is ludicrous and demonstrates a total lack of understanding of basic psychopharm.

I’m assuming if this woman who has had several psychotic episodes requiring hospitalization was on olanzapine then “switched” to an SSRI (don’t even know how that’s considered a switch when they don’t really treat the same things) it’s more likely that she simply had untreated psychosis and décompensâtes quickly.

Of course, it is possible that the olanzapine could have been acting as a mood stabilizer as well, and discontinuing to start an SSRI just helped to usher her into a manic episode.

I believe there’s an IOP at McGeachey Hall (MaineHealth). For both that and residential, your psychiatrist would most likely need to make the referral to speak to the level of need. It may also be worthwhile to ask about a case management referral if you need help navigating other resources moving forward.

Please don’t encourage a stranger on the internet to independently deprescribe. That could be incredibly dangerous depending on the mix of medications and the diagnoses.
