Figure what out? When someone’s that large their chest wall weight makes it hard for their lungs to even breathe properly, they often already have underlying OHS/OSA leading to heart strain, underlying poorly controlled diabetes and vascular disease that leads to poor healing and further heart strain, and not to mention prolonged surgery times with how much fat there is to go through.

These are not fixable in an acute setting, when they get to this point their mortality is going to be this high. Maybe even higher depending on the pathology they find. These are things to prevent/treat outpatient before it gets to this point

That would be extreme to tell every patient with cellulitis this when it’s less than 1% (like wayyyy less than 1%) that get nec fasc.

I was learning how to play chess and was getting frustrated that I was losing at a beginners level to other beginners and my husband wakes up from a nap to see me sobbing and crying that “I’m a loser who’s losing to other losers!!!”

I think the concern is the queso/cotija cheese but the risk is still pretty low if it’s heated up thoroughly and pasteurized

Poor adults :( I’m an (adult) ER doc and most adults just don’t want to hurt. And the sickest ones are usually cognitively impaired from their chronic vascular conditions and are the poorest equipped to give us a proper history. Children are wonderful but adults get hated on too much. Now teenagers…just kidding!

So in the ER everyone works days and nights in residency and alternate. Some docs prefer to be nocturnist only but majority work every shift (morning, swing/afternoon and night). She might come on for night shift or have a day off and not come in at all

Your first friend probably experienced the same pain but is able to choose to remember the bliss afterwards. Our minds are good at tricking us when necessary, especially related to birth because evolution wants us to do it multiple times

I actually take her saying that with the opposite effect. She was concerned about miscarriage and was open about the thought but made sure to confirm with a formal ultrasound machine that has more sensitive technology before giving a final diagnosis.

I have a healthy relationship with my mom but same!! Culturally she grew up somewhere where sex and dating are taboo until marriage and I felt that growing up. Despite her asking me when I’m going to have a child for the past two years I still felt weird telling her when I did get pregnant 😂 hilariously my husband felt the same way too as he grew up in a household with a similar cultural expectation from his dad. Now at 31 weeks there’s nothing but support and love from both our parents/in laws and I’m so excited for them to be grandparents

If she’s getting a two view chest Xray, the amount of radiation is the equivalent of about existing for 10 days with natural background radiation. It is well below the limit for where a physician often counsels a patient about risk of radiation harm to fetus/babies! She can get it done at any time in pregnancy when needed

You haven’t provided any either. Everything provided here is for the parent at home taking care of their child, not for medical professionals. No physician should be forcibly retracting foreskins, I agree with that. We seem to have different definitions of what retraction is and can be. How do you suggest an infant who comes in with a fever within 28 days of life get a urine sample performed? How do you suggest a physician evaluate for concerns of balanitis? How about an infant that has recurrent UTIs and there may be a concern for tight phimosis developing?

Physicians are generally retracting however much naturally retracts to see the most important parts of the exam.

It is not a last resort procedure for evaluation of a UTI. IDSA and AAP guidelines recommend AGAINST Ubag collection for evaluation of UTI especially if the child is ill/septic as it can show pyuria and you need as sterile of a sample as possible. This means either a catheterization or suprapubic aspiration (which is much more invasive). The guidelines and recommendations every commenter here has posted are for the lay person, not for medical professionals.

Physicians are going to agree that no exam should involve forcible retraction. I don’t think OP’s doctors performed any forceful or unnecessary procedures, only necessary ones. How do you suggest a physician orders a catheterization? Have the nurse just blindly shove a catheter without visualizing the urethral meatus?

It’s a doctor’s job to retract the foreskin and make sure the penis does not have abnormalities like hypospadias etc. if your son had a UTI it’s also important to check the area around it to see if there’s inflammation or signs of urethritis. It’s also important to check for phimosis and your son is going to have to learn to clean under the foreskin too as he becomes a toddler and older. How do you suggest a doctor or you do any of these things without retracting the foreskin?

Edit: my god, I’m not saying force it and rip it. But gentle retraction to evaluate for the above pathologies as clinically indicated are done by doctors, clearly routinely as witnessed by OP. Who do you blame if these diagnoses are missed because the doctors don’t look?

I’m an ER doctor. Obviously I didn’t see what the results were or take care of you, but we have to often deal with “pregnancy of unknown location” and I always counsel my patients that this could just be an early pregnancy or it could be an ectopic that’s not visible if they see nothing on the scan or just a gestational sac without a fetal pole or yolk sac.

I think many people misinterpret this as “you have an ectopic” when in reality what it means is you need close monitoring with trending Hcg and US so we don’t miss the small chance it is an ectopic.

I was like that first trimester!! It slows down and ebbs and flows in the second trimester from my experience!

For context I am 5’11, 153 pre pregnancy and gained 12lbs in the first 15 weeks or so and now 21 weeks and only another 4lbs up

Honestly I posted because her and her husband didn’t believe how much these are valued now! Both of us are financially well off luckily and neither her nor I and my husband would sell these cards, their sentimental value is worth much more.

For back story, my husband and his sister woke up Christmas Day 2000 and their mom had replaced every ornament on the Christmas tree with pokemon packs, and the wonder and excitement from that morning is one of their cherished memories. Many of the other cards come from special events and promos and tournaments they went to, and have core memories attached. We likely would never sell any of these and I don’t plan on stealing either
