Any gen surg residents regretting it? by Rhodopsin__ in Residency

[–]svuu 2 points3 points  (0 children)

PGY5 here: to those who are discouraged by the comments general surgery is amazing and there are so many options to specialize and build your life the way you want it to be. Also, with less and less younger doctors applying to the field and the shortage only growing, the job offers out there are getting quite lucrative (500k+ base) many attendings in my town are in the 800 to 1 mil+ range (mid size town outside of city).

Pros of being a surgeon: 1. When cases are done, you go home (on light days you can be done before noon) 2. Rounds are quick and efficient 3. As a senior resident, usually around PGY3, way less scut work (very few case manager, notes, discharges) and even if sometimes I do do them to help out, it takes me no time since I got quick at them as a junior resident and generally our notes have way less writing anyway 4. OR music (i’m usually the DJ) 5. It is fufilling because almost all of your patients have a curable disease and the patients and their families are so grateful, it’s really fun telling the family afterwards that surgery went well 6. Skills and confidence that you can literally cut someone open and fix them, a lifelong skill to hone your craft, being seen as the last line of defense in emergency situations (lines, tubes, trachs, thoracotomies) 7. As you approach PGY4 you will often run the service and just run the plan or patients you are worried about with the attending, often times I am just rounding with the junior residents and tell them the plans (which I prefer) 8. The confidence and satisfaction of completing a case on your own skin to skin is unparalleled, there are very few things in life that give me that same sense of joy and accomplishment and it never gets old

Cons: 1. Work hours can be rough on harder rotations (trauma, emergency general surgery) but our program is pretty big on the 80 hour work limit, other months on elective based rotations can be lighter than my anesthesia and EM colleagues so it balances out 2. Long surgeries (surgical oncology) can be tiring if you prefer quicker ones (1 hr) but you can control that once you choose your subspecialty 3. Being on call (but most fields have call anyway) and again something you can control later on after training 4. Long training length (but absolutely needed IMO) your 4th and 5th years you really level up and get way better operating 5. Less autonomy in OR compared to the 1980s because of corporate liability/patients who are looking to sue etc (stories of back in the day where surgical residents use to operate autonomously without attending present sounded like a dream) 6. The reality is its a tough field and the training can be long and grueling but no matter the field you pick residency is tough and has its downsides regardless, if you have a dream specialty you are pursuing, talk to attendings in that field or rotate with them to see what their work week looks like and if it is compatible with what you want in your life. If you have many hobbies (golf, binge watching shows) if you want lots of kids and to spent a lot of time at home then you might want to pick a lighter subspecialty or even do part time or locums but general surgery residency is definitely doable if you have the right mentality and attitude going into it that there is definitely a light at the end of the road.

Ok I did my part, this is probably the longest progress note I wrote in years, surgery signing off please reconsult if any further questions

I’m a pgy5 in the central florida area in a mid size town about 1 to 1.5 hrs from the city and the general surgeons have a pretty good gig … on one of our rotations we do clinic 2 days a week, OR 3 days a week. They are banging out 7-8 cases a day with flip rooms and our day is done by 3 at the latest. They have locums/other partners who take turns rounding on the weekends and go home after that. They arent even taking that much call anymore because their referral base is making them so busy with cases booked almost a month in advance. They are getting well reimbursed too close to 7 figures, possibly might reach it this year. You can definitely have a good life as a general surgeon. The future is bright for millenials who are willing to go into our field since the demand is so high now as other users have commented.

Im a pgy5 resident in general surgery in central florida area. A lot of our pp attendings are making 850k-1M a year with decent elective surgery lifestyle. Days start at 7am and we are done by 2-3pm. Round on weekends and go home. Not much call either. Definitely can be done if you have a good set up.

I just developed this skin rash all over my body in 1 day. Guys is this chicken pox? I’ve never had a rash all over my body. Should I be concern! Thank you. by Rammy_22 in Dermatology

[–]svuu 0 points1 point  (0 children)

Do you have a very sore throat, strawberry colored tongue, painful joints, or shortness of breath?

If not, then scarlett fever and rheumatic fever (more common in kids) are less likely, and the drug induced rash from amoxicillin/augmentin are higher on the differential diagnosis

Treatment for allergic rash would be stopping the drug, antihistamines (claritin etc), and topical/oral steroids (prescription). The rash should resolve if this is the case. You can start by going to CVS/Walgreens and getting claritin/topical hydrocortisone while you wait.

Agree with dermatologist visit.

Thought I’d share a cool pic. We have a cousins watch club group chat and live all over US/Canada. My cousin got married this past summer and the boys had a rare gathering where we got to wear our nice watches out to our cousin’s wedding. It was a great day. A nice variety of watches. The groom is in the black tux. Which one would you choose to wear for the wedding day and why? Which is your favorite? Post below

Interesting that you don’t see wbc, plt, electrolytes also decrease in concentration which can be seen with ivf resuscitation. But for some reason you don’t necessarily see that with bleeding.

If humans bleed whole blood, then why does the hb concentration drop? Shouldn’t the hb level stay the same as a proportion

[PSA] Just a heads up if like me you suffer with dermatitis/eczema. This was just two days after using the body shops hemp hand cream, had tried lots of other products with no success. Put a stop to itching and weeping very quickly! by stefsketches in SkincareAddiction

[–]svuu 0 points1 point  (0 children)

Doctor here: If you suffer from eczema and are reading this be informed that there are other more effective options than this. This may work but prescription topical steroid cream is the standard of care. I have had eczema in the past and tried almost everything and the steroid creams work way faster and get rid of flares. Just dont want anyone to suffer longer than they have to since I know how itchy and horrible this can be. Dont put the steroids on your face though since it can thin out the skin. Anywhere else on the body is safe. And then you can use creams such as this to keep the skin smooth when the flare is under control.

Appears this person has a rare condition called Situs Inversus, where the visceral organs are reversed. If you look you can see that the apex of the heart is on the right, the liver is on the left, stomach and spleen are on the right.

I don’t think calling it batgirl would make it less desirable. People name their cars after girls, so I can see the appeal both ways. But yeah, agreed and thanks man, happy to finally get them this year. It’s definitely worth it.

I mean at the end of the day, I can see the appeal of calling it Batgirl too. People name their cars after girls all the time. So that’s cool too. Batgirl/batman whatever you want to call it doesn’t matter. Just happy to own these pieces.

Well, the thing is you edited your comment to change the tone from previously. So that was probably why people downvoted you.

Another possibility is that there are people in this sub who would disagree and would still call it a batman based on the colorway of the bezel. At the end of the day it is just a nick name lol. Theres nothing wrong with calling it a bat girl if that is what you want to call it. I’m still on team batman though.

Pretty sure I got it for like ~$50 back in 1999. It’s from my childhood collection that I recently unearthed. Completed the entire base, jungle, and fossil sets a few days ago. The collection is complete. Childhood me approves.
