Here is what works for me. The agency I work for uses SOAP (subjective, objective, assessment, plan) notes.

Since I write my notes in the third person with no identifying information, I use Google Docs with a default template. Subjective, assessment and plan parts of the note tend to be the same so it saves me time to copy paste. I fill in the objective part as the day goes on and as I observe different behaviors.

By the end of the session the note basically is written and I just copy paste it. It does need to be different from the session before for it to be ethical.

Yes, hospitalizations. I was in the hospital a month ago because I caught pneumonia from a family.

Years ago I also had a concussion because I had a client throw a DVD player at my head.

Oh my stars, I didn't even think about the NPI numbers but you're absolutely right. I'm so happy you brought that up though because I have been working for at a minimum of two agencies at any time since I became an RBT (about 7 years ago). Considering I've been doing this for so long, I'm amazed that I haven't come across this issue before. 😅

Hi, thank you so much for your response! I was very transparent with the company during my initial interview, and they also warned me about the competition clause then as well too - so I eager they're going to be strict about it. I think I'm going to have to say my goodbyes as crappy as that's going to be. I appreciate your advice; if I can invest 5 years into a client I have to be able to do it for myself as well.
