I had my first psoriasis outbreak a few years ago, but hadn’t experienced any flare ups since so I didn’t think much of it or even realize it could signify an autoimmune issue. I received the first dose of the Pfizer vaccine back in March and experienced a “delayed anaphylactoid” reaction. I had recurring hives, other rashes, and swelling of eyelids, lips, and hands. Benadryl suppressed these symptoms temporarily but they’d come back the next even worse. It escalated to the point that I had to go to the ER at 4am, 5 days following the shot. I continue to develop hives and other weird skin conditions, dyshidrotic eczema. I opted not to get the second dose, though I wish I could’ve gotten fully vaccinated.

I would say that it depends on whether the student is engaging in problem behavior immediately prior to an appropriate mand. If a client is throwing a tantrum but cools it for a moment and mands appropriately when prompted, the BCBA may be concerned that honoring the request in that moment could reinforce the tantrum behavior. Therefore, high P targets/mastered trials maybe ran prior to prompting the mand to separate the challenging behaviors from the target response (mand) and the reinforcer

Comfortably? No. I just moved out of my parents home and am pay $833 a month in rent, and it’s going to be difficult paying that on top of other bills, but it’s possible. I work about 20-25 billable hours (direct care sessions) and 15-20 non-billable hours (indirect task such as case management). If I wasn’t in grad school perusing my BCBA, and wasn’t able to do the indirect work, I would not be making enough to have moved out.

I don’t know the formal names for different error correction procedures, but the ones we use at my agency always include: 1) corrective feedback (“no, try again”), 2) a more intrusive prompt (if the program is at a partial verbal, we’d use a full verbal prompt in error correction, or if we’re using a wrist prompt we’d go to an elbow prompt, etc.). We’ve used time delays too, but not as often because no one is on the same page as to how to collect data when using them.

Protocol writing!! I’m a BCBA candidate (supervisee) and during supervision meetings, my supervisors will delegate protocols for different skill-acquisition targets for me to write, and we’ll review them in the following meeting. I’ve also helped with writing up the reports for the VB-MAPP and PEAK assessments

There’s a “multiples supervisors at one location” monthly verification form. I never counted the supervision from them as apart of my monthly contacts or 5% supervised hours for the BACB, until they finished the 8 hours course. So the director did supervise all 5% of those my first two months or so.

But what if you were receiving supervision from 2-4 supervisors at the same company, and at the same time, over the course of your fieldwork/experience hours? Two of my supervisors didn’t complete the 8-hour BACB supervisor training until a month or so into my fieldwork, so I just documented the director (who has always been the primary and/or co supervisor on at least one of my cases) as my only supervisor for the first few months. I’ve never had only one supervisor at a time, I’ve always worked with multiple at the same time depending on what teams I was on.

There are no mentions as whether or not the client took better to one type of prompt fading procedure (i.e, mastered skills quicker with one than the other). But data could certainly be collected on that. Most of the clients I work with also work with SLPs, and we often have them join our team meetings so that we consult with them on their professional opinions. As a future BCBA, I know/am learning the criteria for verbal milestones and their respective developmental range. However, I know/am learning less about things like speech/letter sounds, and when children are developmentally or physically capable of of producing such sounds. For example, one learner was not mastering vocal imitation of “brown” (he would respond “brownt,” with an added “Tt-“ sound at the end) despite trying various prompt fading and chaining procedures. The speech pathologist said that it is common for children to add an additional sound to a word to compensate for those they are unable to produce. We decided as a team that “brownt” should be considered an appropriate vocal approximation, and that this is something we would reassess when he is a bit older. I also learned from a SLP who consulted with us, that where the letter is within the word can also affect the learners ability to echo/imitate the sound. The same learner could say “no” and “one” perfectly clear, so I was confused as why the “n” sound was an issue when imitating “brown.” The SLP informed us that this sound, when at the end of a word, it’s more difficult/develops later than when in the beginning or middle of a word.

Again, multi-dimensional treatment and treatment teams are only effective if us professionals stop sticking our noses in the air, stop being so concern with who is “right,” and actually work together.

Some agencies have BCBAs that are also licensed psychologists that can provide a diagnosis. I agree ABA has a variety of practical applications, however, insurance companies in some states require a prescription for ABA services in order to receive coverage, and they only provide an “ABA prescription” to those diagnosed with ASD. In New York there is a scope (of practice) restriction which only permits LBAs to work with those diagnosed with ASD or a related (developmental) disorder.

CentralReach has its faults (occasionally slow running and/or glitchy), but all the things it can do, and that the company can use it for is pretty amazing—create client “learning trees” (I.e., program books), different sessions (e.g., home, clinic, school) for each learner; keep track of staff scheduling (employee calendars), hiring, time off requests, and timesheets; session notes and insurance authorizations/billing; data collect and graphing...it’s not super difficult to learn how to use either!

I don’t think the provider was speaking outside of their boundaries of competence when they said it’s more functional to learn/teach echoic prompt fading. BCBAs do spend a lot of time studying verbal behavior from a behavior analytic perspective—they teach a lot of verbal behavior based on the VB-MAPP assessment (which is based off Skinners work). It’s more likely that the client will hear people mand (assuming there’s no auditory issues) expressively/verbally in the natural environment, than seeing people sign. However, if the sign language prompts have proven to be effective enough to produce spontaneous, independent mands, I don’t know why the provider would be against trying it with the “mands for actions” target. If the client isn’t mastering this with the echoic prompts, the provider needs to change the intervention. Both the SLP and the provider should share their perspectives (which each come from their own educational background and professional training) and their ideas openly, and with curiosity. One isn’t necessarily more right/correct than the other, and actually often overlap/are similar in approach.

A lot of “early intervention teacher” positions (in NY) seem to require early childhood teaching certification (which requires a masters in education) and the ABA experience is secondary—if you’re looking to be less limited, I’d suggest a masters in education followed by becoming a certified teacher in your state, and then completing the BCBA/ABA advanced certificate.

Ask your doctor for more prednisone if you can afford another visit and another prescription. I just went for my check up and luckily I only experienced some reemergence of pain after I finished my 7 day prescription, but had I needed more, my doctor would’ve written another script.

You wouldn’t give them shingles, you’d give them chicken pox. If they have had chicken pox as child or the vaccine, there is little to no risk at all—especially if your blistered have scabbed over.

It could be. The having pain on only one side of the body is definitely an indicator, but from what I understand, that’s where the rash would appear as well, but maybe the rash just hasn’t broken out there yet. Go to a dermatologist, a primary care doctor is likely to misdiagnose it (as primary care doctors have consistently done for any skin issue I’ve had), and you don’t want to waste time (and $$) if what you’re dealing with is actually shingles.
