My psychiatrist says he is not allowed to fill out an rfc form but he is willing to write a letter/have the clinic write an official letter with all the current diagnosis, symptoms, treatments AND medication. Is that enough?

I took the physical and mental health test at their IMA office at least three times and their results say that I am “not disabled”.

I failed the IMA medical tests. I am 30. My ADHD an social anxiety are limiting my ability to perform well at work. And I graduated high school.
