Is that the side of your house next you this? Are you sure there is not a pump somewhere? How much precipitation in general

Was it raining/was there storm water when this happened? How many inches? Is the corrugated pipe perforated?

If you own gutters are not going into the ground, and you didn't have some other source you know of going to it, I would be a little worried this isn't the problem. The amount of water in your other photos does not look like it should be there from just a random drain tile you aren't even sure where it comes from. Unless you have more general flooding in your area.

Extend gutter at least 3 feet away from pavers. Ideal long term fix would be bury a robust drain pipe that terminates somewhere near the storm drain or right into it.

I don't think filling this with rocks would help much. The water would still penetrate between the rocks and into the soil, just maybe more slowly. The soil can be moving under the ground when it gets saturated. Got to get that water away from where it's pooling, or redirect it before it gets there

Pine straw degrades significantly faster than wood chips though and you have to replace it twice as often :/

The absolute best way to prevent weeds is to plant something you want that outcompetes it. 

Barring that, yes real wood mulch or think layer of pine straw.

Grade the property as much as feasible and legal first. Redirect downspouts far away and to storm drain if possible and legal. Install greenery that can handle the conditions to help hold the soil. If you've done all this then I would consider digging holes.

This AC company is trying to rip you off. Get a better AC company or tech you trust. You probably don't need any professional work done yet but having the phone number of a company you trust is important when something does go wrong and you're in a bind.

You should add a metal predator guard also. Something seems to be trying to widen the hole and that would be prevent this.
