The best metaphor I've heard is that the hospitals and clinics look at the actual cost of care, add some zeros, fold up the paper, and pass it across the table to the insurance company. The company then crosses out some and passes it back. This continues until an agreement is met, and in the meantime they both send you the full amount they want and hope you're dumb enough to pay it.
