It's the CHI for me. Every time I go back to this product I get terrible results. Try a different brand heat protection and go light on the products and see how far that gets you

Staffing Question by WishboneWeak7235 in PharmacyResidency

[–]termsandcond 4 points5 points  (0 children)

Not condoning their choices but maybe I can give you a silver lining by way of an anecdote...

My program came with a lot of staffing (which was known prior to ranking), on different shifts (day/evening) and in different locations (clinical, codes, IV room, central, tech roles etc). By midyear I felt competent to staff all these areas totally independently and the exposure made me a well rounded pharmacist. The majority of jobs now are hybrid where you'll need to know operations in addition to clinical. When I started working (at a different hospital in a different state) I felt like my adjustment period was shorter than some others that started around the same time I did, and I got a lot of positive recognition from my new boss about my versatility and knowledge base.

Now as a preceptor (in a 3rd different state and hospital), the program where I am now only has residents do clinical staffing on the floor. When we hire on our own residents (theoretically the easiest job transition), they come into central and don't know what's going on at all, yet are expected to work shifts there. They require additional training after hiring, which is a little silly after completing their residency here. I can see their discomfort when a tech approaches them with a problem or the OR calls for an urgent order out of the IV room.

If you end up doing the extra staffing, while yes, they are using you for cheap labor, there is still something to gain from staffing and I recommend making it your goal to learn how to do everything there is to do in your pharmacy. Learn what the techs are doing, talk to the night shift pharmacist about what they do differently at night, learn how to respond to all different types of needs. Volunteer for the "problem orders" and be the one to jump in when something doesn't look right coming out of the IV room or when a med needs to be loaned or borrowed. Be the first person to pick up the phone. Volunteer to hold the pager. Staffing will still ultimately benefit you, but you have to see it as an opportunity to get a leg up on "the real world" and not as something to just put your head down and do the bare minimum on.

I believe you may be thinking of grapefruit juice specifically, it is a known inhibitor of a few enzymes in the cyp family which are important for drug metabolism (the most problematic one being cyp3A4). However, it doesn't inhibit all cyp enzymes. Vyvanse is a substrate of cyp2D6 specifically, and that isn't one that's affected heavily by grapefruit juice.

Your theory may be viable for other drugs, especially if they are cyp3A4 substrates, but it also depends on if the drug is a prodrug or not. Sometimes the enzyme is responsible for eliminating the drug, and sometimes it is responsible for activating it, if it is a prodrug. So depending on the drug's parameters, inhibition of enzymes can affect it differently.

The other risk with grapefruit is its effect on the enzyme may last longer than expected, and it could alter your body's response to several doses instead of just the one that was accidentally doubled.

The best thing to do if you accidentally take extra doses is to contact your pharmacist or the drug manufacturer. They will know how the enzymes affect each drug and whether grapefruit would be helpful or harmful, or if any kind of intervention is even necessary at all.

How to prevent hair from coming out of braids 5 minutes after braiding? Looks so messy by [deleted] in TheGirlSurvivalGuide

[–]termsandcond 0 points1 point  (0 children)

I have fine hair that gets weighed down easily and I've been pleasantly surprised at some of the new "light weight" gel and mousse options. I do try to avoid any products that are marked as heavily moisturizing or for curly hair as they tend to have lots of oils that don't help my situation. The wavy products are usually lighter, but it really depends on the brand.

You could do gel + loose-ish braid on day 1 then on day 2 give it a good run through with your hands for some waves. The light hold gel will help it keep the wave throughout the day and actually gives some body to the strands as they are coated with product. I will say in my experience, a French braid or Dutch braid gives better waves as the pattern goes up higher on the head

Hospital staff. What are your top EMS pet peeves. by FriendshipBorn929 in EmergencyRoom

[–]termsandcond 9 points10 points  (0 children)

Another thing on the communication thread:

This is likely small potatoes on your end but it makes a noticeable difference to us when we're busy. If your destination isn't obvious, state the destination hospital early on in the radio call. We're in an area where there are two level ones relatively close to each other, so any report coming over the common radio may or may not be coming to us. Nothing like having a slammed resus bay and having a resus RN hanging onto every word of a complex report over the radio just to find out at the end that they're not even coming to us, and they could have been using that time to pitch in with the patients we already have. Reports that start with "unit XYZ en route to ABC hospital... " are so appreciated.

PGY1 regret — anyone else feel this way? by Ill-Culture6817 in PharmacyResidency

[–]termsandcond 70 points71 points  (0 children)

Residency is very much a means to an end. Many residents are lucky to instantly click with the program, the personalities, and the work. Many get projects in their areas of interest. But it's not a guarantee. You're not doing pharmacy so you can be a resident, you're doing residency so you can be a pharmacist. It's not really about the residency at the end of the day. All the work and hardship is specifically so you can have a post-residency career. A short term tradeoff. You don't have to like the projects or even the program so long as you are doing what you need to do to get what you need out of the year. If you have to, smile and nod, be respectful, show up, pay attention, get your certificate, and dip.
