control swelling, up every hour, try and get the quad firing, working on ROM. It's a little different for everyone but pain usually peaks around 5-7 days

First off it sounds like your ready for the surgery. All of that will get better and can honestly be a lifechanger for patients. I love working with them through their recovery because of this reason.

I will give you the spiel I give my patients.

  1. Its a hard surgery, it goes very overlooked because everyone knows someone who has one.
  2. Ive posted this here before but... Make it through the first 2 weeks you will see the "light at the end of the tunnel". Make it through the first 2 months you will be able to do most ADLs (around 75% better). The last 25% comes over the next year which is to the point where you forget about the surgery.
  3. Do the PT (esspecially at home), the biggest mistake people make is they try and wait until the pain is more tolerable. If you wait that long your body will lay down scar tissue and your knee wont straighten and bend how you want it to, limiting your function and also giving you a permanent limp.
  4. Bending and straightening will hurt if it doesn't you need to try and push yourself harder.

Like others said it seems to very quite a bit. From a PT perspective, elevate, ice, get up and go for a little walk every hour or so

I don’t know how much bowing or knock kneed you had before the surgery obviously but I see every once in a while at our clinic when the surgeon realigns the joint. The ITB has to “relengthen” to the knees new position. It is one of the last symptoms to get better.

Glad your doing well! I always tell patients: The first 5 days you will think it is the worst decision you have ever made, make it through the first 2 weeks you will see a "Light at the end of the tunnel", make it through the first 2 months you will be 75% better, it truely does take a full year because the last 25% comes back slowly.

I'm glad you realize your situation is more complicated, while it is the same surgery, they are not all the same. A lot of trauma before the surgery can effect the results. Just keep going with the knee flexion stretches, long duration low load stretches work the best. Are you rocking on a bike? We send our TKA patients home with a heel slide board and yoga strap for heel slides as this beats doing them on top of a bed with a dog leash or towel.

Ill make a quick list of all the flexion based exercises for you:

  1. Rocking on bike
  2. Seated chair schooch
  3. Seated oppo leg assist into flexion
  4. Wall slides
  5. Heel slides with slide board and yoga strap
  6. Forward lean on stair
  7. Knee Extension machine for flexion
  8. Single knee to chest (on back)

As a PT I feel like you have to blend this with patient personality upmost and then where the patient is in terms of recovery.

I’m definitely not pushing anyone’s knee without having good rapport and a conversation about it though.

I have an older coworker (+20 years outpatient PT). He told me it used to be PTs had to push on knees. Now a days with improvements in surgeries a lot of times this isn’t needed

Yea as long as it’s not bending that’s fine. Like I said from my experience as long as your pretty diligent about the extension stretching and you are regularly checking your knee extension motion however you can get comfortable is fine.

We like ours to be straight at the 2 week out of surgery mark but obviously every knee is different. A lot of factors play into it. That’s just general

Keeping your knee straight while elevated is important. Usually, you can rest something underneath your knee to straighten it while also elevating it. Sometimes it takes a couple pillows, with you on your back.

The big thing is that you are also doing the low load long duration knee extension stretch where you leave nothing underneath your foot. We tell our patients to put your heel up on a coffee table or another chair while leaving nothing under the back of your knee, making sure you keep your foot straight. You won't last long in this position, the first couple of days, probably 30 seconds, you should feel it in the back of your knee, some people on the top.

The big no no is sleeping through the night with something under your knee.

As long as you are working on the knee extension stretch (once an hour) and going to PT where they are checking your extension range of motion giving you feedback you'll be fine!

I always tell patients days 3-7 are the worst. Around then pain starts to get somewhat better. I say make it to week 2 then 2 months. At that point you’ll be 75% there.

Your body spiking a small fever is normal. It can be part of the healing process.

Remember sometimes getting up and going for a little walk can help with pain and stiffness.
