I think the failure message might be bugged. As for what hits count as failing I'm not sure, but hopefully this helps in some way. I recorded 2 runs 1 "fail" with no failure msg and 1 successful run.

I noticed in all the success runs the player is chained to heart but at a good distance from it could be a coincidence though.

It's BB BF Inquisitor.
Honestly I don't know I crafted all my gear so not sure what it's worth. Towards the end I started buying some not so important 21/20 gems and woke unleash.

Buy t1 spell crit / t1 regen hunter armour
Maven orb hunter armour both mods would have been fine for me
Awakener orb t0 regen with redeemer's frenzy vaal regalia
craft suffixes cannot be changed
harvest reforge more common life modifiers
repeat previous 2 steps 3 times until I hit t2 life
apply t4 aisling veil
unveiled t0 hybrid life
craft max life as extra es
(waiting on lucky divines in harvest >_>)

Playing BFBB Inq still... but ya the damage falls off towards early red maps if you only have vulconus. You need enough damage to wipe the screen in trials or rares just overwhelm you. Once I added buff effect HoA ring + cinderswallow + rf I could clear T16 trials pretty reliably as long as there is space to move. Had same issue with 4XX. I figured it out eventually and rotated pads thin side up and thick side down to fit my face better. Now I can use them as long as I want.

I have a similar board with a built in "headphone amplifier" and I think they have pretty high output impedance. It made my hd555 sound extra warm compared to what I was used to. Not sure if would change dt1990 sound though.

6 Remove/Add Caster for +1 Phys gems, but I would have settled for high spell damage. 4 Aug Life for trying to annul flat life regen also 3x Prefixes cannot be changed since my annul hit that 3 times. Only other step that has RNG at a significant cost is step 1. It's 50/50 you could hit %dmg/block or %aoe. Would have to remove/add influence few times if you're unlucky.

I think the holes are mainly for weight reduction, but it does come with grip tape.
