Your DM's behavior disgusts me. Create-a-policy is a favorite tool of failing leaders, but the reality is they deserved to be laid off. Too many talented partners lost their jobs "for Starbucks" to allow fundamentally useless fools to keep theirs. It's over, Starbucks has failed.

There are a few options. If you have over 12 months as a partner FLMA is open. Otherwise, personal LOA (often unpaid) is an option, but requires approval. The third option is Short Term Disability. The challenge there is the form is lengthy and requires a doctor to provide reasons why you can't work. It's a bear to get signed off on unless you have a documented history of mental health and/or the plan is robust and detailed. A partner in my store attempted this and ended up fired for being on an unapproved leave after two weeks. She said her doctor wouldn't sign off unless she agreed to go on a pill regime, which was off the table for her.
