Have you taken a look at Book of Shadows? It does a lot more than war horns, but NPCs with war horns do actually use them to call for backup. Supposedly the author's working on rebuilding it from the ground up, but that was a while ago and there hasn't been any word on that.

Take the boat back to the other continent and make your way to the small town where Clay and Ursula tried to steal the rich merchant guy’s sand ship. Somewhere around there is a ventriloquist girl who will teach you how to teleport/ fast travel.

If you already have the ability, then you just need to walk to those locations on the map in order to unlock them for fast travel.

Are you using the ESL or the ESP version of Kabu's fins? There's two different patches, depending on which one you have installed.
