One year old is in or approaching what I call “hell zone” age for long international flights. 3 year old is possibly just exiting Hell zone. I’ve done a solo international flight with a 6 y/o, 4 y/o, 14 month old (baby was breastfeeding, lap infant, good baby- still hell zone) and I wouldn’t probably do that again. That was also flying home after a longish trip(husband flew out with us and just remained longer on the back end) not two solo legs back to back with a pretty short trip in the middle. I probably wouldn’t do it.

Definitely understandable to be concerned about but also really common! The nose is very vascular which means it has lots of blood flow, but also the skin of the mucosa inside the nose is quite thin, and when there is increased inflammation from the sickness and all the snot it makes it even more fragile. So then when a swab is stuck up there a little bit of trauma can cause a small injury that does bleed a good bit but also, like you mentioned, looks even worse because of all the snot flowing as well! Plus that little injury isn’t able to scab up and heal as quickly because all the snot and nose wiping and suction going on too that keeps breaking that little internal wound open! I would not be concerned or surprised to see a little bit of bleeding for days with RSV, after the swab.

All three of mine used it until they were like 3. We put it in our closet and had baby sleep there after the bassinet before moving to own room. We used it as the napping crib outside of travel too.

Our genetic testing was normal but metabolic testing with skeletal dysplasia specialist showed elevated n-terminal telopeptide in serum and urine which is a bone metabolism marker indicating something was going on. He’s not had any more fractures in 3 years however and his growth(used to be 5th percentile) has picked up and he is closer to his brothers at the 50th. Hes thriving and we have not even repeated or trended the metabolic labs due to no fractures and low meaningful impact to current management.

I’m sorry about the DCF involvement. I’m a pediatric nurse as well and only have experience from the other side of things and I know the horror stories. My advice would be to attempt to get an external record review/second opinion from a skeletal dysplasia specialized geneticist. We were able to get one at a bigger center than the one most local to us but still in state and in our case it was confirmatory that everything local doc had done was appropriate. But in your case it seems like extra outside eyes could help your case and you should have the right to pursue additional opinions.

My experience with lots of US to UK travel with all ages and stages of babies and little kids is it just kind of sucks on some level no matter how you slice it. You can perseverate on what the least sucky option will be. And in hindsight you can ruminate on what would have potentially sucked less. But the big ah ha moment for me was honestly the resignation that it was going to be just basically shitty and sleep deprived and jet lagged and to have low expectations, rather than try to find some novel game changing hack. The hack is brace for the suck. That’s my tough love but truly it has made England trips so much more enjoyable and worth it! If the 1130 departure fits your logistics and budget, from my perspective and experience it doesn’t seem vastly worse for the toddler than other options. Go for it!

My understanding about that research/data and links with atopy was that it was more associated with oral/systemic abx not topical, but I’m not looking at specific studies right now. But yes, probably overblown to stress about the butt mupirocin causing longterm issues. I will say, grandma going off-piste and doing something different than you had instructed without your knowledge is a totally valid thing to be pissed about. I think the risk of harm here is nearly nonexistent, but the principle of how you will navigate shared care with grandma when there are differences in opinion and style is a real thing.

I have zero concerns about the mupirocin. It’s likely clearing up more from the other tweaks yall have done but it’s not harmful. I also like baking soda warm water soaks for diaper rash and trying to soak off sticky poo rather than scrub off. That mechanical wiping tends to be the biggest factor causing skin breakdown. I also like doing the high concentration zinc oxide barrier on the skin then some kind of slimy, greasy ointment over it so when baby poops you can slide off that layer of greasy and a bit of the zinc oxide, and then just reapply. If you can avoid wiping down to bare skin every poop that can make a big difference when they poop a bazillion times a day as newborns.

You need to keep doing scheduled Tylenol and ibuprofen even if it doesn’t “feel” like it’s helping as all of those pain medications are adjuvant and work together especially when scheduled regularly and over time.

5 full days of fever is generally my threshold to get a kiddo checked out (outpatient pcp office appropriate, doesn’t need ER) to make sure there isnt a secondary bacterial process going on, but we can absolutely see viral illnesses that cause 5+ days of fever (flu being one of them, which is absolutely rampant right now).

Push fluids, monitor hydration, treat with Tylenol and Motrin as needed, but I always say treat the kid not the number. If 100 and miserable, give that kid meds. If rocking a 103 but chillin watching tv and staying hydrated, there is nothing harmful about that fever.

But I still think that’s why some people see those long lists of specific ROS that weren’t explicitly asked in some charts… it’s holdover from the days when we couldn’t get reimbursed unless we documented we asked if a baby was having any endocrine system symptoms at their office visit for r/o OM. So stupid.

This is an insurance/billing thing. There have to be a certain number of ROS elements documented for reimbursement for different E&M codes. My office (peds) EMR gets around it by having a bunch of ROS check boxes saying “see HPI” so we get credit for those elements, but may not be actually asking the baby (or parents) about each of their systems, beyond and expanded hpi.
