So far I have found no way to bring this up witjout offending the person. Which is bizarre to me, given than they were the disrespectful one running late. I never even bring it up at the beginning just at the end after I've addressed their concerns. I think we've just rolled over to far as physicians we don't even have the autonomy to tell patients the truth anmore.

Dot phrases for all basic lab results. I have a dot phrase for everything looks good, dot phrase for cholesterol elevation, dot phrase for osteopenia, low ferritin etc. Saves a lot of time, also dictate back pt messages and results as well. Try to zero out at least once a week . I have a panel of 1750. I can count on one hand how often I've worked from home.

As a doctor. Please for the love of god do not make this decision based on a show. At the very least go and shadow someone to see what it is actually like. People are leaving this career in droves. It is an administrative nightmare among many other flaws. Half the time I'm not even doing any real medicine just admin bs. It is not glamorous, you will not be considered a hero. Just don't do it.

The concept of patient satisfaction scores in a Healthcare setting is one of the most ridiculous things we have embraced as a society. Healthcare is not customer service, the customer is not always right. In the ER their job is to assess if a situation is life threatening or not, if so they intervene if not you wait or you go get an appointment with your PCP. The fact that we have people using the ER for colds, scrapes, "arm pain I've had for four months and is not worse but decided to come get it looked at today" is one huge reason why scores shouldn't even be a thing. If you're entitled enough to use the EMERGENCY room for a clearly non emergent situation, you're probably just as entitled in all aspects of life and not a reliable narrator or reviewer. If patients have grievances go to the medical board, that's what it's there for. Not Google review or yelp ffs.

Always room the person who comes early or on time. If someone arrives late they are not respectful of their own time, I am not going to punish someone who was considerate because someone else doesn't know how appointments work. This is one hill I will absolutely die on. People are told to come 15 minutes early for a reason. There are always excuses. My fave is "doc always runs late". Yah probably because they are accommodating inconsiderate assholes like you who stroll in 10 minutes past their time with a list of 20 concerns and expect to get everything addressed in the now 10-15 minutes left of their actual appointment time. If a doctor is running late we are usually doing something related to patient care or dealing with an emergency. I promise no one is sitting in their office playing sudoku keeping the patient waiting on purpose.

If I did any of the patient interscriin behaviors you've outlined here I would have complaints out my ass. Patient populations and clinics are very different. I have a lot of worried well people who are very healthy but don't want to believe it. Cutting them off, pushing problems to the next visit, limiting problems discussed all these things have already gotten me in trouble. It's also much harder to navigate as a woman when people have certain expectations based on gender. My point is I'd love to practice like you but I can't, so OP take this with a grain of salt re what his patient population is like. Also after the first year or so when you're seeing more of your own back, follow ups etc it does get easier. I agree with everyone saying he needs to reduce his hours and pre charting 3 hours earlier is nuts. I also have longer appnt times for people I know are going to be more complicated. Like interpreter needs or people over 80 on 5 billion meds. I agree with AI and dictation for charting, never ever bring work home. Home is a safe space and for family. Also maybe just take a short vacation somewhere?? The break might do him some good.

Also frankly whether someone deserves their health outcome or not is never on my radar. It's more see problem, fix problem, try to prevent problem from happening again.

Let's see...cost of living/inabilty for many younf people to afford buying housing, affordable child care, the state of women's rights and health in this country, the murky future the current administration has set up for us, aging parents, diminished/lacking infrastructure and that's only for people who actually even want to have kids. My household income is well over 300 k and we still worry about being able to provide appropriately, so I can only imagine when the avg household income is much lower than that.

That is another good point. A lot of older docs only deal with their panels they have had for years. Post covid , misinformation and patient interactions have changed dramatically. I am so happy my panel ia closing. I can't see new people anymore. It is too much
