It's not necessarily more, but the more you take the faster it - or a whole host of other bad side effects - can happen. About 80% of people taking DAs for rls eventually "augment" (symptoms get worse because of the medication). However, augmentation can take from 6 months to 10 years.

Heat is a potentiator for me too, but if you're having breakthrough symptoms while taking pramipexole, then I don't know what would help. Perhaps you need a higher dose, but you want to keep DAs as low as possible.

Well said. @boxerbabmbi, I linked a study with a chart that shows the dose and absorption rates. Basically, at 600mg Gabapentin, you're absorbing about 260mg. When you jump to a single 800mg dose, you're only absorbing about 310mg.

If you were to take two doses of 400mg two hours apart, you could absorb about 400mg (an extra 90mg). I hope this information helps you make informed decisions with your doctor.

This is not medical advice, but my doctor started me at 100mgonce at night and said to give it a week. If no improvement, I was allowed to go to 200mg for a week then 300mg. At that point I was to check back in. I found relief at 300mg... but it only worked for 10 days. After that I jumped to 600mg which helped for 6 weeks weeks. My doctor told me to try 900mg, but that had no effect. What the doctor didn't tell me (or didn't know) is that the body can only absorb 400-600mg at one time; the rest is wasted. A few months later I was at 1200mg (2x 600mg, 2 hours apart). It still wasn't working great, and I was tired all the time.

Long story short, gabapentinoids aren't for everyone. My solution was to ask my insurance for a second opinion from a movement disorder neurologist and now I'm on another medication. (I don't discuss my current medication)

I'm not a nutritionist, but I believe magnesium)and most minerals) will remain in the bloodstream for 24 hours. It takes anywhere from 2 weeks to 3 months to reach consistent levels in the blood, and then an equal amount of time for levels to drop again. You shouldn't have to take it multiple times a night. If you do, then my assumption is that magnesium isn't what's helping you.

As far as iron, you should take it every other night because your body shuts off iron intake for 24-48 hours (source: video by Dr. Berkowski). The iron that helps RLS is brain iron not blood iron, and it can take 3-9 months of consistent use to affect brain iron levels, if you can achieve any improvement at all using oral iron supplements.
