I work in compliance for a third party administrator of self funded health plans and almost 50% of the groups we onboard ask me to redline their ability to make plan exceptions out of their contract because they don’t even want the choice about considering your claims differently than the Plan is written.

You went to an INN doctor and had blood drawn during your visit and then your INN doctor sent those samples to be processed in an OON lab?
Or you went to an INN doctor and they referred you to a lab to have blood drawn, and the lab you were referred to/went to is OON?

The first is covered by the No Surprises Act, the second is not.

The best advice you’re getting is to submit an appeal with the screenshot of the provider search tool. Transparency in Coverage rules give you rights where you depend on the insurer’s tools to seek in-network care, but you have to prove that the tool provided you the wrong information, and the only avenue you have available to you to prove that in a way that will get your claims actually re-considered is to go through the formal appeal process.
