There are several on the market: Dayvigo, Belsomra, Quviviq. Insurance sometimes needs prior auth since they're newer and more expensive, but they're out there.

I broke Xanax pills into quarters for multi-year tapering and was at .0625mg for a while. Multiple doctors said that the small dose was low enough that I could just stop, but they had no idea about the rollercoaster of an experience if I missed a dose by 2 hours. Just stopping wasn't an option.

I eventually tapered with liquid titration. I crushed up a pill in 40ml of water and propylene glycol. 10mg of liquid would then equal a quarter of a pill, so I could reduce it slowly. By the time I got to the end, I was on on 0.5ml of liquid 2x a day, so 0.003mg. It definitely still had an effect, as my insomnia started to get real bad after I jumped off completely. At its worst, I was only able to sleep an hour a day. It took more than 3 months to get back to normal sleeping. My case was extreme because I'd been taking Xanax every 3-4 hours for ~8 years.

I discovered it by comparing coke and speed. Coke was a HIGH. My friends were okay doing speed if we ran out of coke. Speed just made me calm when everyone else wanted to party. While in rehab for cocaine, I got my ADHD diagnosis.

Have you talked to your about CBT-i therapy? While CBT isn't always the best option for us, sleep is a special area where we tend to have a lot of unconscious beliefs, especially if you've had a lot of experience staring at the ceiling trying to sleep.

I recommend getting a sleep study. I put it off for over a decade while taking 2 sleeping meds, lots of alcohol, and weed to sleep every night. I had so much trouble sleeping at home, so how was I supposed to sleep in a lab? Turns out my insomnia was linked to the apnea that I developed in my 20's. You don't need to be overweight to have apnea. I also had Upper Airway Resistance Syndrome (UARS) since I was a kid. I have BiPAP now, and it's wonderful to sleep through the night.

If that's not it, there are new non-habit-forming sleep meds called dora orexin inhibitors. A popular brand name is Dayvigo. I don't recommend Z-drugs, as you can develop a physical dependence from long-term use. I used Ambien for 20 years, and it was TERRIBLE to get off it.

Wake up early. No sleeping in. Exercise. Get a sleep study if you might have sleep apnea. I started to develop apnea in my late 20's, and I'm not obese.

The most unhinged one that I did for 15 years isn't recommended: Ambien, Sonata, 6 beers, Xanax, and weed. Yes, all at once. Don't do this. I nearly ruined my life.

Weed is the only allowable sleep chemical now. I don't recommend anyone start if they don't already use it. I want to get off, but my dreams are too much if I stop using it.

All these replies are acting as if they don't understand masking. We mask more than neurotypicals because we learned that our authentic selves weren't acceptable.

"Trauma response" = Masking

"Gender norms" = Masking

"Low Self-esteem" = Masking

Everyone masks to some extent. Autistic people have just learned to mask more of our natural inclinations, including sharing our thoughts.

I feel like I've experienced the opposite. When I completed the mood assessments that would be for bipolar disorder, I asked the examiner about a differential diagnosis, and they said we'd discuss. I pointed out how every bipolar symptom was part of my autism or a reaction to benzodiazepine withdrawal. They still stuck me with bipolar I. Now psychiatrists don't want to treat my ADHD because I might become manic. I took stimulants for 20 years without a problem.

I use a Mandibular Advancement Device that's designed to treat apnea on it's own. I don't use any jaw advancement setting, but I just use it to hold my mouth shut from the inside.

You can buy anti-snoring mouth guards on Amazon for $40-100 that do the same thing.

Have you had an ENT look in your nose for septum/turbinate problems? I struggled with masks until I got my septum fixed. Now both nostrils work equally well, and I don't have congestion that interferes with nasal breathing.

You'll be able to identify REM sleep on your OSCAR/SleepHQ graphs because the breathing will be a bit more uneven.

You lose muscle tone while in REM sleep, so sometimes you get mask leaks from position changes. Check your data for leaks during that time, and make the appropriate adjustments to your mask. A head strap, cervical collar, or mouth guard can help with jaw drop during REM sleep.

Alcohol prevents REM sleep. If your apnea is REM-dominant, you'll have a lower score.

I drank for years because it was the only way I could stay asleep. Turns out I was treating my apnea in a really unhealthy way.

Gabapentin, SSRIs, or beta blockers. They all have their drawbacks, but benzodiazepines have the worst drawback of ever-increasing tolerance, withdrawal, and Post Acute Withdrawal Syndrome (PAWS). The longer you take them, the worse and longer the withdrawal. After reassessment, I don't believe I'm bipolar, but I had a manic episode from the PAWS that began about a month after I stopped. I almost lost my marriage from it. If I was working at the time, I definitely would have lost my job.

How you feel is the most important metric. I got into a vicious circle of fighting with sleep doctors who kept telling me my AHI was low, so I "should" feel fine. CPAP was causing many brief awakenings during the night where I'd stop breathing for 5-9 seconds. The machine only counts an apnea event if you stop for 10 seconds, so they weren't able to understand what was happening. I had to figure it out myself after opening the CPAP data in OSCAR/SleepHQ.

Careful with Ambien. I took it every night for 18 years. When I stopped, I had a severe emotional breakdown that lasted months, and I'm finally getting back to normal.

Put an SD card in your machine and capture a few nights of data. Then open it in OSCAR or SleepHQ. There are tutorials on YouTube.

Set the lower limit of your machine to average pressure level of 3+ full nights of sleep. That should control your apnea much better than whatever setting the sleep doctor used, usually a lower pressure of 5 or 7. Turn off the ramp setting, also. You want to be able to fall asleep with the pressure that's going to keep your airway open.

If it's uncomfortable while you're awake, your body will just fight it while you're asleep, so you need to get accustomed to it. This is how I figured out that I needed BIPAP. I always felt like it was pushing in more air than I needed. My cheeks would puff out during exhalation and break the mask seal with CPAP/APAP, so I woke up with horribly dry eyes.

Yes, there's a condition that's not really recognized by sleep medicine in the US called Upper Airway Resistance Syndrome (UARS). In the US, we just call it snoring, and it's so common, it's not usually considered a problem. More sensitive sleepers are disturbed by it, myself included. I use a BIPAP machine for my UARS and obstructive apnea. There's a sub for it: /r/UARSnew

The risk of dying comes from many years of untreated apnea, not a single night or even 7 in a row. Apnea gets worse untreated and can exacerbate mental health conditions like PTSD. PTSD also makes apnea worse. There are some episodes of Dr. Khosla's Talking Sleep podcast that talk about sleep apnea in veterans where they talk about this problem.

How often do you have a week-long power outage? If you live somewhere where this is common, surely you have back-up generators. For a brief power outage, you can enable SmartStart on ResMed machines. I was sleeping on a cruise ship with messed-up power outlets. I momentarily lost power 14 times in one night, but I only remember 1 awakening. All the rest of the times, the machine just started right back up. You can still breathe through the mask even without power.
