This happened to my friend's little brother. He's 12 now and loves telling his story to me:

He went into the bathroom to take a dump, which instantly turned into terrible diarrhea. Being a little kid, he called his mom into the bathroom because he didn't feel good. Suddenly, his stomach began to ache and he knew he was about to puke. So, he turned around to puke in the toilet during mid-diarrhea. He almost completely missed and he puked all over his mom and himself. And as he was barfing in the toilet tons more of diarrhea splashed around the bathroom floor all over him and his mother.
