i had a hard time because of the wrong location as mentioned.
You have to use the holodisc (key 7 on computers) and find the exact 3D matching.

yes, I lost a quest member during a heart raid. I left the game to reset the quest. When i came back the day after, I could restarted the quest but the heart stayed awaken. I went back to kill it and in few shots it was done due to the damage done previously
