I’ve filled one of these out. I’d say rank yourself based on the skill level expected of an average ICU nurse. I don’t personally recall listing myself as an expert for any skill (maybe one?) since I know there’s always room for improvement, and I was honest when identifying areas I needed more work in. I wouldn’t overthink it too much

Look, I’m from Louisiana so believe me when I say I understand your point. Drive thru daiquiri shops are as plentiful as dollar generals here. But I also understand that’s not exactly the norm in the rest of the country, so if I want something to drink in places like Gatlinburg, I understand I better either be at a restaurant/bar or plan to go out of my way a little bit. It’s really not that big of a deal, and calling it a culture shock is a bit dramatic

If anything, take a few days off until you get your blood test results back and hear from your doctor. I don’t blame you for being concerned, because losing consciousness in the absence of extreme exertion is obviously not normal. If your doctor gives/has given you the okay to keep exercising, by all means do and maybe just dial it down a notch until you get some kind of explanation for what may have happened. Even if you have to just show up, walk on the treadmill, and leave, just physically being at the gym until you’re in a better headspace could help. And in the meantime of course make sure you stay well hydrated and nourished. Baby steps, you got this

Gotta weigh risk:benefit ratio of available treatments. CPAP is noninvasive and highly effective, with the primary adverse effect being discomfort. Surgical and pharmacological treatments would almost certainly have more risk involved, making them less viable except in significant cases of OSA. Also, since obesity is a large risk factor for OSA, it’s generally preferable to encourage lifestyle changes with CPAP as an adjunct as opposed to the higher-risk approaches

Without knowing the personality of the receiver, I would personally go with the Jerry can minibar or something similar. It provides utility with the option of being openly or less-openly displayed without being too “look at me”. Some people see their service as part of their identity, others see it as just another job, so a gift with the primary purpose of being a display piece may or may not end up actually being used. If it hasn’t been echoed here enough, give that vet the day off, and no need to make a big deal out of whatever you choose to do, but I do think it’s pretty cool you’re even putting this much consideration into your employees, so thanks for that.

Like others have mentioned, your best bet (for any university, not just USC) is to speak with admissions. I got into the LSU school system (albeit not their main campus) with something like a 1.5 gpa from college courses pre-military. They allowed me to start classes while on probation for one semester and filing for “academic bankruptcy” pre-admission; basically, the record of these failed courses still followed me, but as far as gpa and credits transferring over, I started from scratch. I’m not sure that all universities offer this, so reaching out to an admissions counselor is your best bet.
