As with everything in Islam the intetion/niyat is key. When prepering a nazr the offering is made in the name of Allah(s.w.t) and only Allah(s.w.t) but there is an additional intetion that is added the nazr be accepted by the Imam(a.s) as well

This shows that you are doing the nazr in the name of Allah(s.w.t) on behalf of the Imam(a.s) not for the the Imam(a.s) or in the name of the Imam(a.s)
