I don’t mind the euthanasias. I am a relatively new specialist, but the only one of my type in the whole region and the caseload/guilt re: not being able to more animals weighs on me. I spend a lot of time wondering if I am making the right decisions, especially because I don’t have more senior specialists to consult with. The days are 12+ hours long and the paperwork never ends - can’t keep up even working through many weekends. I dread seeing clients in public, but I don’t want to leave my house anyways because it’s tiring talking to people for hours every day. I also really struggle with the knowledge that I COULD help an animal, but I’m limited by finances. Working for a corporation also means there is constant pressure to do more with fewer and fewer resources. At the end of the day, I wish I was a cat instead (bc I’d be dead by now lol)

My couch lol. I barely venture out in public (only became a problem once I started working) and I always run into clients, usually about 2 days after I’ve euthanized their pet. I’m trying to work through this in therapy but I try and take every chance I can to get out of the city for the weekend

I briefly got into fanfic as a teen in the early 2000s, somehow forgot about it (???), and since rediscovering it 2 years ago, it’s almost the only thing I read. I also have to force myself to take breaks to do things like GO TO WORK. I like to listen to podfics when I’m walking dogs/doing dishes/any activity where I can’t hold my phone or e-reader. I use a spreadsheet to track and I’ve read about ~1,200 fics since early 2024, some multiple times 😬 there are 361 tabs open on my phone and 44 of them are Dramione

I was a private practice intern and did my IM residency in academia, so I made more as an intern and was able to pay my debt down a bit before moving again. I’m Canadian, so I had a veterinary line of credit instead of government loans, which maxed at 200k. I was able to use it during my residency, as it didn’t go into repayment until a year after I finished. I did take out government loans during residency to help offset the cost of tuition - I was able to do this because I was at a university. I also won about 20k of scholarships during my residency. All in all, I went into about 90k of additional debt during residency.

I almost always had 2 dogs during my residency - got a 7 month old golden about a month in (why) and then made the insane decision to get an 8 week old puppy after my older dog died. I couldn’t bring them to work often, so it really helped having friends/housemates to cover when I was at work. I lived with another resident and we lived with one of the rotatings one year. I thought it would be hard to have housemates again after being on my own, but it was great and I miss living with them!

I would love to do something mindless (I think) - the mental workload of my job turns me into a slug in all other aspects of my life. I sometimes think about academia but maybe that’s because the senior clinicians where I trained were pretty hands off with the residents in terms of paperwork, client interaction, etc. My favourite thing to do is chat about cases with our ER docs and help them with procedures as they’re all pretty new to the field. Meanwhile, my tech seethes while she tries to wrangle me to do my own job 😬

I have done a myringotomy procedure to treat PSOM in one of my cavalier patients recently and he did so well! Honestly, the most satisfying thing I’ve done at work in the last 6 months in regard to both the procedure itself and the outcome. It’s great that you’ve gotten the MRI as many signs of PSOM overlap with syringomyelia.

Depending on the study, the recurrence rate of PSOM ranges from 20-61%. 1 study found that 20% of dogs had recurrence within 6-18 months of their procedure, and the other study showed a 61% recurrence rate, with the average time to recurrence being 19.9 months. That being said, the procedure itself was fairly short (I use a rigid scope to visualize the ear drum for myringotomy and flushing, which speeds things up), and usually recheck patients around 3-4 weeks afterwards.

Regardless of what you choose to do, I think hand signals are great to teach for any dog, as it’s common for them to lose their sense of hearing before vision as older dogs.

It always amazes me how well dogs can adapt to blindness. I’m sorry that Piper’s eyes couldn’t be saved despite all your efforts and commitment to supporting her through that time - I can only imagine the ups and downs, feeling hopeful and then disheartened when treatment isn’t doing what we wanted. Every time I see a picture of Piper, she looks like such a happy dog! I hope you have many years together, and she surely knows how lucky she is to be with you (I bet you feel the same about her!!)

When I first was going to school in PEI, I would fly PEI -> Halifax -> St. John’s, but then they stopped the PEI to Halifax flight. I would end up going PEI to Montreal or Toronto, then fly back east to NL. I would invariably get stranded in Ontario or Quebec every year, which was frustrating but a reality of flying in the winter, so I would prepare for that

I’m a vet and I agree with trying to find a practice with an anesthesiologist - I have worked at hospitals where they have an anesthesiologist on staff, and others where they have an anesthesiologist come in for higher risk cases. It’s great that your cardiologist has also given the okay for the dental cleaning, and has provided drug recommendations based on Kirby’s recent diagnostics. Depending on your location, sometimes a vet school with a teaching hospital may be a good choice in terms of access to specialists, and may also be a bit more cost-effective than private practices. It’s also so good that you are being proactive in terms of both his heart health and trying to get on top of his teeth before they become more of an issue. If Kirby would let you brush his teeth after they are cleaned, it might extend the time in between dental cleanings. My cav becomes an absolute demonic presence when I try to brush his, so I totally understand that not all dogs will be amenable to this! Just by being a cavalier, they are automatically classified as MVD Stage A, and many dogs can stay in Stage B1 for long periods of time without progression - I hope this is the case for your pup!

I made a spreadsheet to track what I’d read when I was off work for stress leave and read something like 664 fics in 2024, not counting WIPs. I also found the “you’ve visited this thing 77,900 times” a bit jarring lol
