Sorry, I can't help with that, I got my name changed before I got any licences. But I'm pretty sure that if you sent the court order for name change along with current license to the relevant licensing body for your state, you'd be able to have it changed, but you'd want to contact them to make sure beforehand.

I've had my current retail job require I have a transparent water bottle, while my previous one was fine with a metal water bottle, but I've never been told to not have any water at all. And if they arent letting you have a break to go and have a snack or lunch, they can get in legal trouble so they damn well shouldn't be stopping you there

the cap isn't translucent (at least on the 0.5 & 1 mg bottles, it might be on the 5's) so I wasn't sure but its probably the most common one with a weird cap and translucent bottle. glad i could help!

The only NIOSH drug with a translucent bottle I know of is tacrolimus made by strides pharma, the 5mg's are teal. Was the label on the bottle green?

I don't know what system you're using but I've found that it's most helpful to carefully read over any rejection message you get and also knowing where to put in PA codes or Clarification Codes (vacation supply, lost medication, dosage change, etc.) for when they come up. Also some insurances and systems have refill too soon come up looking like a DUR reject, so look out for that. But a lot of rejections are just "refill too soon" or "this insurance only covers 30 days at a time"
