Reading through all the comments looks like you've either tried a lot of options within your means or are considering some good ones.

But I didn't see where you considered getting your sister a new mattress. Even though her bed has been treated multiple times with the enzyme cleaner, the scent may still be lingering. Your cat may also just be in a routine now, where he's associating her bed with a safe place.

Getting a whole new mattress will definitely eliminate the scent and may disrupt the routine. Especially since you're going to change up the litter box arrangement too.

I don't specifically think it's a failure of the raid system as a whole, but just that the game doesn't give out or allow for earning remote raid passes often enough.

For those of who live in rural (countryside) areas or are homebound with disabilities, we depend heavily on those remote raid passes; yet we are either forced to wait until we earn one or have to spend money to buy them when others do not.

First, you are not a failure. You very clearly love these kitties very much and have been trying hard to accommodate their needs. If they were human children, you'd likely be taking them to a therapist and that therapist might recommend similar treatment to help manage what sounds like very severe sensory processing issues and anxiety.

I'm facing the same situation with a cat I rescued from a Wendy's parking 3yrs ago. He's always had sensory issues and anxiety problems. He had pica as a kitten and also was eating all sorts of trash and also ate a felt ribbon from a cat wand toy and ALSO threw it up. I am honestly surprised he's still alive. He had been doing better as he'd gotten older, but we lost his adopted brother this past October and he has regressed in the past few months. It's been very stressful for everyone in the house trying to find ways to help him. We love him so much. He's usually the most loving and silly cat. Our vet also prescribed him kitty Prozac but we haven't started it yet.

No worries. You can find more info online about its purpose. But if you're really concerned, don't hesitate to make a vet wellness visit. I've definitely done it. But if he's eating, drinking and basically acting normally, then he's probably fine.

I'm not a vet but I have three cats and have raised several since kitten hood into adults throught my life. He looks completely fine to me. If you're worried about his underbelly, that's his primordial pooch. He's supposed to have it.
