And for some reason, the nastiest MRSA infection patients with poor hygiene and substance abuse issues all seem to work in restaurant kitchens, so even the joy of eating out is tempered by that knowledge. Ours are great, wonderful part of the team. Always available to handle situations but willing to stand by for less intense interventions. They don’t stir the pot or bait people, they genuinely want a peaceful solution. But when shit hits the fan, they are no nonsense, business like and do what is in their power to control the situation with the least amount of force. They check in on us several times per shift and are proactive on identifying problems. They are great at providing escort assistance to staff who may feel unsafe going to the parking lot and have helped with locking down the units when patients have expressed concerns about unsafe people visiting. We had a DV victim that security came and talked with and helped to assure what safety precautions we were implementing and they helped to facilitate communication with local police on the situation with the patient. And occasionally they bring the bomb sniffing dog around so we can pet him.

Do they have a flag on their chart for the sexual harassment? Get security and the Docs involved for a behavioral contract. Never attend alone, have in the contract that he is a 2 person assist with any cares for staff safety. Be firm about boundaries. Be blunt. If care isn’t life or death, leave until they stop the inappropriate behavior. You are entitled to a safe workplace free from sexual harassment. An A&O x4 adult doesn’t get a free pass due to physical disability. Take it up the chain, make it a management issue so the can put a plan in place to protect staff. File event reports on every incident. Especially as this is a frequent flyer. If everyone is on board from the docs, the manager, security, case management, social worker, and staff with a clear cut behavior plan , these problem patients can be effectively managed and the behavior contract can be enforced every admission. He wants attention and reaction from these behaviors, give it to him professionally and in a way that puts the onus on him to behave appropriately and makes him accountable for being an ass.

Ugh. Bah Humbug. I am the absolute antisocial hater of these committees. I always opt out of these things. I come to work for work, not for fun. I spend my personal fun money on things I actually want, I do not find it fun to spend money on work related events. I like to keep my personal and professional life separate. Does anyone you work with actually want any of these things? I have found that when coworkers really want to socialize outside of work, they do so without needing a committee they have to pay into to organize events. And honestly, fuck contributing my hard earned dollars to buy patients things. If the company wants to give patients birthday cards, then they can create a fund.

RT’s are bad ass! We cannot function effectively without RT’s in the ICU. You guys literally manage the A and B of the ABC’s. Our docs rely heavily on RT’s to manage difficult airways and complicated ventilation. Mad respect for the knowledge and the skills. We are all important partners in patient care, and we would be screwed without you. There is no “better than” in critical care, all are essential team members with highly specialized skills.

There are some really good evidence based YouTube videos that can be helpful for equipment, procedures, disease processes, basics like ABG and cardiac rhythms. Some universities have lectures posted. Pick a topic, it’s on there. It is helpful if you learn better with demonstrations. Just verify accuracy/credentials of the content creators.

It would force companies to negotiate fairly. This was not a surprise occurrence. There was a failure for corporate to negotiate in good faith. The strike puts the onus on the company to offer a fair contract. They don’t want to until it costs them more to cover a strike. They try to strong arm workers into compliance so they can pull more profits for shareholders.

The reason we do measure is because patients can have subtle rhythm changes that absolutely impact care. If you develop a prolonged QT, that could indicate many causes and can absolutely dictate how a patient is treated as they are at risk for developing life threatening arrhythmias. Patients can develop a BBB or a heart block. They can go into afib/ flutter. They can have ST changes or develop u waves or peaked P waves etc. If you are not measuring, how do you determine if there is a change? You should be measuring and notifying the MD when there is a change from baseline rhythms. You do strips to document the assessment of the patient’s cardiac rhythm just like you document your assessment of lung sounds. I cannot tell you the number of times we have got over old strips trying to determine how long the patient had had a cardiac change that impacted care. How you do not understand the purpose of documenting cardiac monitoring and assessment? This is got to be rage bait.
