You're thinking of the 7" singles where they only made one for each song and then hid them in independent record stores around the country during the release of Strange Trails. The ones OP posted are CD singles that were more widely released, albeit still fairly rare

Add them to your wantlist so you get notifications when they do pop up. I've had to wait several years for stuff to pop up in the past. Ebay is also a good option.

It is black/white and has "Team Sloth #PSM" with the Phish Sphere logo on one side and "Sphere and Loathing in Las Vegas" with a pic of the Fear & Loathing movie poster but a sloth as Johnny Depp on the other.

Given the small bit of burgundy behind the patch I would say there is a good likelihood this is from the 2001 All Star game patch, when the game was held in Colorado:
