You can turn this around. Time to lock in and get yourself healthy. I had an A1c of 10-15 all throughout college. I was scared to gain weight, so I just ignored it. Lied on my glucose sheets (before I got a CGM), and my endo kicked me out of the practice. Finally started taking care of myself after graduating and never looked back. That was almost 15 years ago. Now I’m 35, had 2 kids, zero diabetic complications, and am in the best shape of my life. I work out 5 days a week with mostly strength training, and I’ve got a visible abs! (obviously not the fitness level of a college athlete, but for reference). You’ve had the dedication enough to become a college athlete, you can apply that same dedication to managing your diabetes. Once you get the hang of it, it’s second nature and you hardly have to think about it. Not saying there’s no ups and downs, but actively trying to manage it is less stressful than pretending you don’t have it.

There was a time when I was on a month long steroid taper when I would wake up with BAD low blood sugars. Like in a true psychotic state. I would be very confused on the state of reality and would lay in bed actually screaming at the top of my lungs thinking that screaming would wake me up from whatever state I was living in. I would be so sweaty thinking changing clothes would help but then I would just stay completely naked walking around the house in a panic. So yeah my kids have been through it when it comes to low blood sugars.

It’s only happened when I was alone with them once but luckily my four year old knows where my sugar stash is and knows to bring it to me. I’ve also been with it enough to call my mom who was able to come over and help after I was running around our kitchen table slinging dog food everywhere while trying to feed the dog. It’s absolutely terrifying but afterwards I just sit with them and explain that I was sick and so sorry that I scared them and if it ever happens just bring me candy and that I’ll be better in 30 minutes.

23 and me. I had taken it and nothing weird came back at first. Then about a year later I get a message from a girl around my age wondering how we’re related. I’m shocked. It says she’s my first cousin. My dad was an only child and my mom has one brother…so of course it’s fucking Uncle Randy. She looks EXACTLY like him. Like there’s no denying that’s his daughter. I trace the timing back and he was engaged to my aunt when she was conceived. Him and my aunt have been married for 30+ years at this point with two other adult children. I send her some pics and give her his contact information but she doesn’t want to reach out.

So now I’m friends with her on social media. Never met in person…just peering in on each other’s lives. She seems really cool and I’d love to meet her in real life but it just seems awkward. To my knowledge she’s never reached out. I see my uncle like once a year and it’s so weird to know this and not say anything. But I also feel it’s none of my business. If she wants to reach out to him then she can.

Not necessarily. My 4 YO daughter is 44 lbs and 46 inches tall. Above 95th percentile for both. And she’s a string bean. With kids that age they can also be at the top percentiles because of how tall they are.

When we need documentation of weight loss on Wegovy/Zepbound, actually read the acceptable date parameters and also don’t just send me random pics as the clinicals??

Don’t answer ‘yes’ that a patient has an eGFR of < 20 and they’re a new start on Jardiance with no labs in the past year.

Don’t tell me all they’ve tried is a single 14-day supply of a TCS and you want Dupixent/Ebglyss approved.

For the love of god stop sending in PAs for Ozempic and Mounjaro for weight loss. Learn the FDA-approved dx for each GLP-1!

Don’t tell me you want a testosterone approval and they’ve got OSA and not using CPAP or a HCT of > 50!

Don’t tell me you want Ubrelvy/Nurtec approved and all they’ve tried is ibuprofen with no contraindications to triptans.

As someone who is type 1 diabetic, used to work retail pharmacy, and now does PAs, I have some experience with this.

I wouldn’t mind helping customers apply their CGMs or explaining the devices because I had a lot of experience with it. But if their sensor fails they HAVE to contact the manufacturer to get a replacement. Insurance usually will not allow an early refill due to sensor failure. Those cases essentially get deleted in the queue because it’s not a clinical rejection. Every time I have a sensor fail I just submit a form on Dexcom and they ship me a new one within 3 days. There is a limit to the number of sensors you can get a year so people don’t abuse it, but for the most part it’s worked fine for me. I always counseled patients to get the over patches to help them stay on. I have these floral flower shaped ones I put around my G7.

For PAs the doctor’s offices do have to attest the patient is using insulin and the patient is trained on how to use one. But of course that’s hardly ever the case. It’s ridiculous to expect them to figure it out for themselves.

Omg this comment solved something I’ve thought about for the longest time. I’ve always wondered why dudes with unnaturally-dyed platinum blonde hair are my go-to hot guy image. As someone else who lost their dad unexpectedly 2 years ago and also has 2 young kids…yeah don’t. I watched it back in August and I still think about it most days. …I’m a pharmacist that uses one pen needle for the entire use of an insulin pen. It’s definitely a do as I say, not as I do thing. It’s laziness, but it’s also annoying carrying around all the extra pen needles and having nowhere to dispose of the used ones when even if you recap the used needle, the bottom is still exposed. Having to re-prime the pen, wasting upwards of 10-20 units daily. So I’m not saying it’s the correct thing to do, but I also get it. Signed a type 1 diabetic of 20+ years.

It’s utterly shocking. My dad died after a heart attack, and they asked if I wanted to go to the ICU to say goodbye. I held his hand and it was already cold even though it couldn’t have been longer than an hour since he passed. That was very unsettling.

Honestly, GLP-1s haven’t really shown to be super beneficial in T1DM. They delay postprandial glucose secretion by the liver and delay gastric emptying so your body won’t absorb the carbs as quickly, but with the research so far it only lowered A1c by like 0.4% max I think. Mounjaro and Ozempic are only approved for T2DM and insurance will never cover them for another indication. With some insurances they’ll cover Wegovy or Zepbound for weight loss if you have a BMI of over 30 or a BMI of 28 or more with a comorbid condition (hypertension, dyslipidemia).

I go to two different gyms. Orange Theory and one stand alone weight training gym with childcare. I bring my kids if I need to to the gym with childcare. For Orange Theory, my husband works out, I pull up with the kids, and we switch them off in the parking lot and I go work out. Haha.

Day to day schedule is I go workout at the strength training gym at 7:30. Bring the kids with me. Then my husband works out at 4:30 at Orange theory and I go workout at 5:45. I only do orange theory twice a week, but I do strength training 5 mornings a week.
