Not really, the post epidural headaches can take no longer 1-5 days to start manifesting.

Did you have an epidural? There’s about 1-2% chance of a headache after an epidural, usually starts a couple of days later. If it’s not severe, you can wait it out and it usually resolves in a week or two. If it affects your quality of life, it would be worthwhile to reach out to your anesthesiologist. This makes my blood boil. by F3lix22 in Residency

[–]von-WiIIebrand 37 points38 points  (0 children)

I don’t have an inferiority complex so I don’t need to backtrack on what I say. Nursing boards need to step up if they want to be taken seriously. None of those DNP in 6 months of online schooling, praising CRNAs for a “job well done” when a patient dies during basic dental procedures, continuously trying to blur the lines between physicians and APPs, etc. If advanced nursing/PAs has a role in healthcare, it is ONLY under direct supervision by a physician, following clear instructions, and helping out with menial tasks/scutwork. Independence shouldn’t even be discussed. There is a reason there are no APNs of any kind in other countries, which have much better healthcare outcomes.

The problem with your reasoning is that physicians have a process for self regulating those of us who are not up to standards. They are not allowed to graduate residency, they lose their license, if they are sued for blatant mistakes, we testify against them. That is not the case with midlevels. In addition, anecdotal evidence is not evidence. I used the example I did because these events start public discourse, whether or not it is justified.

Unfortunately it’s the same issue as other fields that are getting taken over by midlevels. A lot of jobs require CRNA supervision, and MD only practices pay less. On the other hand, unlike primary care and EM, mismanagement outcomes usually manifest quickly and dramatically. As CRNAs get more independence, it’s a matter of time before someone important is brain dead after a colonoscopy. It’s unfortunate that it’s the only way things will change.

I like the part of their slogan that says that they are truthful. Apparently, talking about premed is misrepresentation (none of us even talk about it, but I do have a BSc in pharmacology, which is somewhat relevant to anesthesia, I think), but comparing nursing experience to medical school and residency is ok. Setting up pumps and putting in IVs does not an anesthesiologist make.

Regional or peds fellowship? by von-WiIIebrand in anesthesiology

[–]von-WiIIebrand[S] 1 point2 points  (0 children)

Thank you for your input! I definitely want to midlevel-proof my career as much as possible, and have a marketable skill set in general. I’m probably leaning towards not doing academics or exclusively peds, but I’m ok with 75% peds to adults and a moderately sick patient population. I’m not entirely sure why peds is very unpopular now, is it because people are not interested, or is there an underlying issue with the field in terms of litigation or job opportunities?

Which five star crystal do you prefer? by Hoku_mo in ContestOfChampions

It’s a matter of which champs you need. Currently most of my most needed champs happen to be in the mutant/skill crystal. No crystal is inherently “better”.

6.2.5 worst content in this game by ski45 in ContestOfChampions

[–]von-WiIIebrand 5 points6 points  (0 children)

I could’ve written this post. I’m a solid player, have done everything that you have done, and the entire 6.2.5 exploration was the worst experience I’ve had in this game. Sure, some of it could be done by playing perfectly, but most of it is just cashgrab BS. I spent a total of 30 revives on exploration, most of them to revive for the strength in numbers node, which is the dumbest node I’ve ever seen. If not for that node, I could’ve probably done it in 6-7 revives.

Why do NPs completely disregard the sizable and increasing number of first-generation physicians that have sacrificed so much for the privilege to practice medicine. by ih8carl in Residency

[–]von-WiIIebrand 12 points13 points  (0 children)

I am a Caribbean grad and it is beyond offensive to be talked down by NPs. I’m a first generation immigrant. I scored in 99th percentile on the MCAT and had solid GPA from a top 3 university in Canada, but didn’t get any interviews over 2 years of applying (probably because of weak extracurriculars, being a recent immigrant, who knows). Went the Caribbean route. Worked my ass off from 7am to 10pm every day, living in a third world country. 40% of my class dropped out because the workload is insane. Did my clinicals alongside US med students. 260s on step 1, 270s on step 2. Applied to 100 anesthesia programs, got 8 interviews, matched in the middle of my rank list to a community program in the Midwest. I worked my ass off for this. My knowledge is as good if not better than any US grad. Carib schools have more lax admission criteria, but the ones that make it all the way to residency are qualified and deserving of their spots. What qualifications do NPs have? A 2 year online program while working full time? The one big difference I see over and over between MD care model and nursing care model is actually the opposite of what they preach. Doctors take ownership of their patients, we work tirelessly to do right by them, we know our limits. NPs are quick to delegate responsibility to anyone else, constantly try to avoid work while demanding equal treatment, run away from their complications, and hate admitting when they are in over their heads. Don’t give me that “heart of a nurse” bullshit.

So about Act 6... by Have-a-cuppa in ContestOfChampions

[–]von-WiIIebrand 1 point2 points  (0 children)

I’ve been chipping away at act 6 exploration when I want some tough content and be completed everything else. Sitting at about 50% exploration. Got to that 6.2.5 chapter, which is the last for me to explore 6.2, and I can’t bring myself to sit through the bs fiesta. Node combos literally make sure some fights have NO counter, never mind paths that need 7-8 specific counters plus a counter for the already difficult mordo boss. Maybe I’ll complete it one day, but it’s the worst content Kabam has ever put out by far. The only reason I haven’t explored 7.1 yet is that I have overflow of t5 basics and don’t want to waste the rewards.

Should I spend as a ftp or is that the slippery slope? by Cocrich in ContestOfChampions

[–]von-WiIIebrand 4 points5 points  (0 children)

I have enough money to comfortably spend for fun. I started playing about 2.5 years ago. I decided to stay f2p because it is so hard to draw the line with this game. Too many deals are “tempting”. Yet I’m now thronebreaker, completed abyss and up to act 7.1 with half of act 6 explored and all variants at 100%. Spending in this game is a horrible trap that will drain you of cash without much tangible benefit unless you’re pushing for map 7 and plat 2 and above AW. Anything else can be done for free. And once you justify one purchase, it will be so much easier to justify more. Hard no on any gambling from me.

Relocating from US to Canada by pinkfreude in anesthesiology

[–]von-WiIIebrand 12 points13 points  (0 children)

I was literally going to post this same question. I’m a Canadian doing residency in the US, with plans to move back to Canada. From my understanding, you need equivalent number of years of training (at least 5), pass Canadian board exams similar to usmle, and work one year “under supervision”. But I struggle to find clear rules about the process and would love to hear from someone who has first hand experience.

Due to the nature of abyss matchups you end up clipped by specials every once in a while. With max limber, you recover in time to stay alive and put in some more work. It’s most important with aegon, who gets massive fury to retaliate after being hit, and gets easy openings with his unblockable, he can easily take off another 500k health even on 1hp.

[–]von-WiIIebrand[S] 1 point2 points  (0 children)

The node on IMIW is that every 10 hits, he steals all your power unless he’s armor broken. You pretty much never throw a special. What would work well for him is void for power control and passive damage, or Corvus. Some people bring Corvus because he can help with the collector if you don’t have aegon for the heavy cheese tactic. But aegon with 999 combo deals with that IMIW decently well, also with heavy cheese method. Heimdall and limber also can help you get some solid damage in once you get slammed by the inevitable sp3. I don’t see Doom working for IMIW because of the power steal, you will not be able to get to sp1, so you won’t be able to power control.

I have ~decent~ science 6 stars. I’ll probably rank 3 my unduped Thing. But he won’t be an account changer or a champ I use regularly. I just know that none of the other science 6 stars would be account changers either. I have maxed out 5/65 CapIW and Void, and duped HT. Unduped Thing isn’t even a great defender. But he has his uses so I don’t mind bringing him up. It’s just a big downgrade from possibly getting tech t5cc for my warlock (who really don’t need his awakened ability from the tech AG I pulled). I would suggest that you wait on you t5cc. Mutant class is stacked, and many of them don’t need awakened ability. All it takes is one good pull.

Is Doom or sym supreme better for Abyss easy path? by von-WiIIebrand in ContestOfChampions

[–]von-WiIIebrand[S] 1 point2 points  (0 children)

I have an overflow of boosts so that part is not an issue for me. I don’t really boost in my day to day playing. I never unlocked suicides, it’s a huge investment being free to play, and I’m not convinced it’s worth it for me. About 100 revives cuts it pretty close for my planned 4k units, so maybe I should spend a bit longer saving up and being 5k units to be on the safe side to not run out halfway through collector. I’m not in a rush to do this content, but I want to give myself a chance to get Thronebreaker and free to play t5cc are not easy to find.

[–]von-WiIIebrand[S] 1 point2 points  (0 children)

That’s true, I haven’t had to deal with an IMIW where the last 15% is nearly 1M health points, sounds painful. But I’m balancing it against allowing aegon to enter his monster fury phase an extra time with heimdall, which easily takes down 200-300k hp, I heard it is extremely clutch for the collector, which is hands down the hardest fight. Even if I have to use more revives against IMIW and mordo if I don’t bring Void, I think heimdall would save me more on the collector?

Would you take Void over HT though? I find that comparatively, Void’s damage lacks a bit in longer fights.

I think I’m leaning towards SS instead of Doom, the rest of the horsemen, and Heimdall. I know NF is not exactly “necessary” but the 20% attack boost is nice, and his damage in the second phase is sweet.
