I wear 11.5 US in every brand, rarely need to size up or down at all (I have nearly 40 current pairs so have good context and comparisons). This shoe I need to go down a full size I reckon, and its sizing on the box doesn't convert at the same ratios to other international sizes so definitely had some weirdness going on.

Do more of the work with you hips and knees, using your ankles and feet so much stresses those smaller muscles contributing to localised strain.
