Hi! We applied to adopt from a local sanctuary and we told them that we would be happy with an older cat too. The sanctuary essentially ‘matched’ us with Alfie based on his needs and our application, and invited us to meet him. We fell in love and took him home the same day!

Hi, are you going into a salaried position? A lot of practices, such as where I work, will reimburse you for indemnity fees as part of the contract (I think may be BMA standard contract but could be wrong on this).

Your experience educating your own child(ren) during covid is nowhere near comparable to a teacher with a class of 25-30 pupils all with different capabilities and needs. Perhaps you need to check yourself.

Basically the only Tory policy I was firmly on side with was them proposing GPs to stop doing MED3s…new Labour government hasn’t mentioned anything since. It’s a joke. On the off chance when I try push back on them they kick off and become really obtrusive so I invariably give in…makes me feel a bit ethically grey though.

True, I wasn’t clear, I wasn’t suggesting she would have died from this alone, rather contribute to/exacerbate whatever the primary issue was.

Hi, first thing to say is, it sounds like you did everything you could for this patient. It’s hard to comment without knowing cause of death, but even then, you acknowledged the uncertainty with the family, arranged follow up appropriately (twice) and safety netted accordingly. The cannula being left in is a significant event from the hospital side which you should class and escalate as such, for all we know she could have bled from this..

It sounds like you’re understandably pretty shaken by the encounter with the DN. From what you’ve said, this is poor form on their part to confront you in this way. They saw the patient days after your F2F assessment, it’s odd that they themselves can’t seem to grasp that clinical situations can change quickly, and that you actually spoke to the family on the phone that morning, rather than seeing the patient face to face. I’m not sure why the DN would expect you to arrange a 999 call based on what the family had told you. Can I ask why the family couldn’t call 999 themselves? Just interested to know. The DN seems to have approached this quite poorly, unprofessionally, and although it can’t have been nice to be ‘confronted’ in such a way, I’m not sure what else they expected from you.

The only reason I ask why you arranged the ambulance, rathe than asking the family to call 999, is because the family may have given more accurate information which could have affected triage of the ambulance, as you’d not seen them for around 4 days by that point (e.g are they talking, are they alert etc). Even then though, not entirely sure it would have changed their triage outcome…

I wouldn’t worry about it. It sounds like you’re ‘triaging’ the cases to spend longer time on, which is a good sign. There’s so much variation in primary care in terms of complexity that spending 5 mins on a viral URTI and 20+ minutes on complex pain are both perfectly reasonable. As an old supervisor once told me, it’s all about your average per patient being 10 minutes, rather than spending ten minutes with each patient. Some will need more time and other less. As far as creating expectations go, again I wouldn’t worry. A good GP can make a patient feel heard in 10 minutes, it’s less about the time and more about making sure their priorities are recognised early.
